In a paper published in Nutrition Journal (Open Access, available without subscription at http://www. nutritionj. com/home), two researchers from SUNY Downstate Medical Center show that low carbohydrate, high protein diets can be expected to be more effective than low fat diets, going against long standing prejudice of the nutritional community, which has claimed that only calories count.

“There are numerous examples of low carbohydrate diets being more effective than low fat diets with the same number of calories. It doesn’t always happen but it can happen,” said Dr. Richard Feinman of the Department of Biochemistry. “The nutritional establishment has been reluctant to accept this, because they say it violates the law of thermodynamics. However, they never seriously look at the thermodynamics, which not only says its possible, but it is to be expected.” he added.

In their paper, Dr. Feinman and Dr. Eugene J. Fine explain that thermodynamics is as much about efficiency as it is about energy conservation. Carbohydrate is an efficient fuel, whereas protein is not. On a low carbohydrate/high protein diet, even though total energy is conserved, more energy is wasted as heat, a process known as thermogenesis. This energy comes from burning fat.

The researchers stress that “the human body is not a storage locker. It is a machine and the efficiency of the machine is controlled by hormones and enzymes. Carbohydrates increase insulin and other hormones that regulate enzymes, leading to storage rather than burning of fat.”

“Of course, people are different” said the authors, “but many people are sensitive to the effects of carbohydrates and for them, a low carb diet is going to work well.”

The practical point is that getting rid of the idea that “a calorie is a calorie” opens the door for serious research into what kind of diets will be most effective and which people will benefit most. “This is important,” they explain “because millions of people

Are seriously trying to lose weight on low carbohydrate diets, and instead of being given directions on the best way to do this, they have been largely discouraged by health professionals and self-appointed expert groups. The obesity epidemic is too important to allow this to happen.”

“The March 2010 issue of Consumer Reports (“Deadly infections: Hospitals can lower the risk, but many fail to act”) addresses one of the most pressing healthcare issues facing our nation today: healthcare-associated infections (HAIs). As a patient safety advocate, the Association for Professionals in Infection Control and Epidemiology (APIC) appreciates the fact that Consumer Reports has chosen to focus on the checklist that many hospitals are using to prevent deadly bloodstream infections and has included summaries of central-line associated bloodstream infection (CLABSI) rates in healthcare facilities.

“Since 1972, APIC has provided thought leadership on infection prevention to drive attention to the critical need to protect patients from HAIs. APIC has established itself as an authority in the application of evidence-based infection control measures and hospital epidemiology.

“In 2004, APIC announced its support of public reporting of healthcare-associated infections as a path to improved quality and patient safety. Further, APIC supported (http://www. apic. org/Content/NavigationMenu/GovernmentAdvocacy/PublicPolicyLibrary/Health_Care_Reform_HAIs_Joint_Letter_July_16_Final. pdf) the HAI reporting language in the House of Representatives Healthcare Reform legislation, which would require nationwide HAI reporting through the Centers for Disease Control and Prevention’s National Healthcare Safety Network. We believe this surveillance network is the best method for ensuring the establishment of a scientifically meaningful reporting and monitoring system for HAIs.

“While not all HAIs are preventable, APIC believes that every healthcare institution should be working toward a goal of zero healthcare-associated infections (http://www. apic. org/AM/CM/ContentDisplay. cfm? ContentFileID=11707). Many of our member facilities have seen that central-line associated bloodstream infections can be reduced to zero, and that in many instances zero can be maintained. We’ve also seen that reducing infections can significantly improve a healthcare institution’s financial bottom line. All institutions should be adopting evidence-based interventions to reduce CLABSIs and ensuring that these measures are consistently applied.

Dr. Dennis Gage, M. D., Endocrinologist, Obesity Expert, and Author describes the ill effects of Insulin in his book, The Thinderella Syndrome: A Practical Guide to Individualized Permanent Weight Loss (Vantage Aug. 2004). Dr. Gage says “The higher the Insulin, the more illness we see.” Women with PCOS seem to get a pattern of Insulin Resistance that resembles the type occurring in the obese type 2 diabetic. Dr. Gage’s answer, “These patients must protect themselves from the carbohydrate environment. Low carbohydrate diets don’t just happen, it takes behavioral planning and preparation. From shopping to dining out, patients need to keep their guard up. With behavior modification, Dr. Gage claims he has seen several patients reverse the syndrome, even without medications.

Unlike other books that describe single methodologies as a mode of weight loss, Dr. Gage offers a custom-made group of behavioral interventions that, when applied, will work for each individual. Through his theory of “The Three R’s of Behavior Modification” (Realization, Regimentation, Reorientation), he describes what is involved in taking back control of one’s eating lifestyle. Hays reports millions across the UK are being spent on coaching without a business purpose and with no measurements in place to assess the impact

Hays Senior Finance reports that while multi-millions of pounds are spent on coaching across the UK, 15% of organisations admit they have absolutely no measurements in place to assess the impact of their programmes.

A new survey from Hays Senior Finance, working with expert coach/ mentors LeaderShape, shows that coaching is often undertaken without evaluating the return on investment. The survey also shows that 68% of businesses fail to use coaching directly to address corporate objectives and outcomes are often simply referenced at a personal level.

According to the data, the majority of coach-mentoring programmes are for individuals, with 72% deployed to develop individual talents or address personal worries.

Chris McCarthy, director at Hays Senior Finance, said: "In far too many cases companies are leaving it to individuals to set the framework for their own coaching or mentoring - with little or no reference to business needs. They then fail to check the standards of their programmes and show little concern for the outcomes. Support can be extremely effective when a professional starts a new role, but it is essential it is carried out in the right manner by people who understand the specific needs of these individuals."

The survey showed that if a senior finance professional leaves within three months of starting a new role not only is there a financial cost, but it dampens staff morale. The vast majority of respondents (96%) agreed that these appointments would benefit from confidential support during this period.

One major concern revealed by the Hays Senior Finance/LeaderShape survey is the lack of ongoing professional development for providers. Internal coach-mentors are the preferred choice by those who use coaching, with 73% of companies using in-house support. However, 16% of coaches and 28% of mentors do not receive any training or support themselves, while companies describe 29% of team or group facilitators as untrained.

Chris Gulliver from LeaderShape commented: "This is a very expensive missed opportunity for UK Plc in fast-moving times. Increasing amounts of money are being spent on coaching as a universal panacea but many companies have no comprehensive overview or sense of purpose.

"There is a clear lack of framework and training given to those who are delivering many of these programmes with the obvious outcome that they simply don't understand how to use coaching effectively and spend money wisely. In what other area of business would money be laid out with so little thought to evaluating its impact?"

Chris Gulliver added: "Coaching is not regularly applied where it can often be most effective - within the leadership teams. And it is clear that many services are being given to staff, often in difficult situations, by people who are themselves untrained and unsupported. We believe it is essential to upskill internal providers by training them in a range of coach mentoring and facilitation options. It is equally important to track observed outcomes against business objectives. Otherwise businesses could waste time and money and miss the opportunity to boost success."

Citing a study which shows that even mild acne can be associated with such emotional problems as significant clinical depression and even suicide, a physician who treats depression and cosmetic skin problems is encouraging parents to seek medical treatment for their teenagers who have acne.

"It’s been found that the cosmetic impact of even mild acne can cause a profound emotional burden for some young people," said Dr. Aaron Barson, Medical Director of Surface Medical Spas, a medical clinic in Layton that treats acne along with a host of other cosmetic skin conditions. “During high-school and college, appearance becomes one of the defining characteristics of how people see themselves. Taking care of treatable cosmetic problems at this time can lead to greater self-confidence and success later on in life.”

The research stems from a Canadian study published in the British Journal of Dermatology. Using a standard 52-item rating scale for depression, psychologists examined the prevalence of depression among 480 dermatology patients with four different skin disorders, including acne. They found that the level of depression and suicidal thinking among patients with mild to moderate acne was similar to that among patients suffering from moderately severe psoriasis, a far more chronic and cosmetically disfiguring condition.

The study concludes that, "The effect of a skin disorder on body image should be assessed in the context of the patient's life and development stage, as teenagers and young adults with acne may have greater difficulty adjusting to the cosmetic problems imposed by the skin disorder in contrast to older individuals with more chronic and disfiguring problems”.

This new research, as well as studies that date back as early as 1948, shows that acne can affect all dimensions of a person's psychosocial well-being. According to the study, the effects may be expressed in a number of ways, including low self-esteem and self-confidence, negative body image, anger, embarrassment leading to social withdrawal and lifestyle limitations. The study suggests that seeking rapid correction of an acne problem may be critically important for some people. Rather than treating the problem with over-the-counter medications and taking a wait and see approach, an evaluation by a healthcare professional can help determine the best treatment method from all of the options available.

Fortunately there is help here now. New treatments that have stemmed from hair removal using light waves have been proven to be effective in treating acne and have recently been given FDA approval.

The procedure lasts about ten minutes and although several visits may be needed, the treatment boasts a nearly perfect record and most patients feel only a mild snapping sensation. The effects are nearly immediate and eliminate the need for prescription drugs like Accutane that have been proven to cause birth defects.

Statement of Need:
There is an unmet need for maintenance pharmacologic treatments that target bipolar depression. Despite the preponderance of depressive symptoms and episodes (versus manic) over the course of both bipolar I and II disorder, and the more frequent functional impairment associated with bipolar depression, there has been a disproportionate lack of attention paid to establishing effective management of bipolar depression. This has recently begun to change, however, with the approval and addition of atypical antipsychotics to the arsenal of medications used for bipolar depression. Recently completed studies and trials in progress are expected to contribute to the growing literature supporting the effectiveness of these types of medications for the treatment of bipolar depression. Clinicians are in urgent need of education and performance improvement in the recognition, diagnosis, and maintenance treatment of bipolar depression. Published treatment guidelines are an adequate framework for developing treatment plans. However, clinicians need additional education on pharmacologic treatments for bipolar depression. In this evidence-based, neuroscienceCME TV activity, experts will provide information on pharmacologic treatments for bipolar depression that will enable participants to integrate these findings into current treatment guidelines to further enhance and maintain good patient outcomes.
