The first and most critical decision the therapist must make is whether to hospitalize a patient with major depression, or to attempt outpatient treatment. Clear indications for hospitalization are: (1) risk of suicide or homicide, (2) grossly reduced ability to care for food, shelter, and clothing, and (3) the need for medical diagnostic procedures. A patient with mild to moderate depression may be safely treated in the office if the therapist evaluates the patient frequently. The patient's support system should be strengthened and involved in treatment whenever possible.

Antidepressants

Studies have show that antidepressant therapy for major depression can dramatically reduce suicide rates and hospitalization rates. Unfortunately, very few suicide victims receive antidepressants in adequate doses, and - even worse - most receive no treatment for depression whatsoever.

One of the biggest problems with antidepressant therapy is that most patients don't stay on their antidepressant medication long enough for it to be effective. A recent study found that only 25% of patients started on antidepressants by their family physician stayed on it longer than one month. Antidepressant therapy usually takes 2-4 weeks before any significant improvement appears (and 2-6 months before maximal improvement appears).

First Line Antidepressants

The SSRI antidepressants, escitalopram (Lexapro), Fluoxetine (Prozac), paroxetine (Paxil), fluvoxamine (Luvox), or sertraline (Zoloft), are considered excellent choices as the patient's first antidepressant because of their low incidence of side-effects (especially weight gain) and their low lethality if taken in an overdose. All SSRI antidepressants are equally effective.

Because many patients with major depression also suffer with intense anxiety, your doctor may also give you Fluvoxamine (Luvox) or lorazepam (Ativan) to reduce anxiety in mixed anxiety-depression.

Both Fluoxetine (Prozac) and paroxetine (Paxil) tend to be stimulating (elevate your mood); thus patients with mixed anxiety-depression can often dramatically benefit from the addition of clonazepam (Klonopin) to the Fluoxetine (Prozac) or paroxetine (Paxil) therapy.

Keep in mind, prescribing the right antidepressant is not an exact science. It may take some experimentation on the part of the doctor (make sure you're seeing a psychiatrist, a specialist in psychiatric medications) to find the right antidepressant and right dosage for you. Do not give up if everything doesn't come together right away.

SSRI antidepressants should be taken for 6 to 12 months. Antidepressant therapy should not be withdrawn before there have been 4 to 5 symptom-free months. Withdrawal from antidepressant therapy should be gradual. Never discontinue taking your medication without telling your doctor first. Suddenly stopping your medication could produce severe withdrawl symptoms and unwanted psychological effects, including a return of major depression.

Psychotherapy

In general, psychiatrists agree that severely depressed patients do best with a combination of antidepressant medications and psychotherapy. Medications relieve the symptoms of depression quickly, while psychotherapy can help the patient deal with the illness, easing some of the potential stresses that can trigger or exacerbate the illness.

Dynamic Psychotherapy

Dynamic Psychotherapy is based on the premise that human behavior is determined by one's past experience (particularly in childhood), genetic endowment and current life events. It recognizes the significant effects of emotions, unconscious conflicts and drives on human behavior.

Interpersonal Therapy

Interpersonal Therapy is based on the theory that disturbed social and personal relationships can cause or precipitate depression. The illness, in turn, may make these relationships more problematic. IPT helps the patient understand his or her illness and how depression and interpersonal issues are related.

There is some evidence in controlled studies that IPT as a single agent is effective in reducing symptoms in acutely depressed patients of mild to moderate severity.

The National Institute of Mental Health studied interpersonal therapy as one of the most promising types of psychotherapy. Interpersonal therapy (IPT) is a short-term psychotherapy, normally consisting of 12 to 16 weekly sessions. It was developed specifically for the treatment of major depression, and focuses on correcting current social dysfunction. Unlike psychoanalytic psychotherapy, it does not address unconscious phenomena, such as defense mechanisms or internal conflicts. Instead, interpersonal therapy focuses primarily on the "here-and-now" factors that directly interfere with social relationships.

Behavior Therapy

Behavior therapy involves activity scheduling, self-control therapy, social skills training, and problem solving. Behavior therapy has been reported to be effective in the acute treatment of patients with mild to moderately severe depressions, especially when combined with pharmacotherapy.

Cognitive Behavior Therapy (CBT)

The cognitive approach to psychotherapy maintains that irrational beliefs and distorted attitudes toward the self, the environment and the future, perpetuate depressive affects and that these may be reversed through CBT.

There is some evidence that cognitive therapy reduces depressive symptoms during the acute phase of less severe forms of depression.

Electroconvulsive Therapy (ECT)

ECT is primarily used for severely depressed patients who have not responded to antidepressant medicines, and who frequently have psychotic features, acute suicidality, or food refusal. It can also be used for patients who are severely depressed and have other chronic general medical illnesses which make taking antipsychotic medications difficult. Changes in the way ECT is delivered have made ECT a better tolerated treatment.

Importance of Continuation of Treatment

There is a period of time following the relief of symptoms during which discontinuation of the treatment would likely result in relapse. The NIMH Depression Collaboration Research Program found that four months of treatment with medication or cognitive behavioral and interpersonal psychotherapy is insufficient for most depressed patients to fully recover and enjoy lasting remission. Their 18-month follow-up after a course of treatment found relapses of between 33 and 50 percent of those initially responding to a short-term treatment.

The current available data on continuation of treatment indicate that patients treated for a first episode of uncomplicated depression who exhibit a satisfactory response to an antidepressant should continue to receive a full therapeutic dose of that medication for at least 6-12 months after achieving full remission. The first eight weeks after symptom resolution is a period of particularly high vulnerability to relapse. Patients with recurrent depression, dysthymia or other complicating features may require a more extended course of treatment.

In a 1998 article, in the Harvard Review of Psychiatry, entitled "Discontinuing Antidepressant Treatment in Major Depression, the authors concluded:

"The benefits of long-term antidepressant treatment in major depression and the risks of discontinuing medication at various times after clinical recovery from acute depression are not as well defined. Computerized searching found 27 studies with data on depression risk over time including a total of 3037 depressive patients treated for 5.78 (0-48) months and then followed for 16.6 (5-66) months with antidepressants continued or discontinued. Compared with patients whose antidepressants were discontinued, those with continued treatment showed much lower relapse rates (1.85 vs. 6.24%/month), longer time to 50% relapse (48.0 vs. 14.2 months), and lower 12-month relapse risk (19.5 vs. 44.8%) (all p < 0.001). however, longer prior treatment did not yield lower postdiscontinuation relapse risk, and differences in relapses off versus on antidepressants fell markedly with longer follow-up. contrary to prediction, gradual discontinuation (dose-tapering or use of long-acting agents) did not yield lower relapse rates. relapse risk was not associated with diagnostic criteria. more previous illness (particularly three or more prior episodes or a chronic course) was strongly associated with higher relapse risk after discontinuation of antidepressants but had no effect on response to continued treatment; patients with infrequent prior illness showed only minor relapse differences between drug and placebo treatment.

Refractory Depression

Refractory depression occurs in as many as 10 to 30 percent of depressive episodes, affecting nearly a million patients. Katherine A. Phillips, M. D. (a 1992 NARSAD Young Investigator) has found that failure to provide adequate doses of medication for sufficient periods of time is perhaps the most common cause of apparent treatment resistance. Once the clinician has determined that a patient is truly treatment-refractory, many treatment approaches can be tried.

Phillips recommends the following treatment strategies for refractory depression:

1. Augmentation with lithium, and perhaps other agents like liothyronine (T3 or L-triiodothyronine) (Cytomel). Trazodone may be worth trying either alone or in combination with Fluoxetine or tricyclics if other approaches have failed.

2. Combining antidepressants - supplementing the SSRI antidepressant with a tricyclic antidepressant. Several studies have shown a good response when Fluoxetine is added to tricyclics and when tricyclics are added to Fluoxetine. It is important to monitor tricyclic levels because Fluoxetine can raise tricyclic levels by 4- to 11- fold and thereby cause tricyclic toxicity.

3. Switching antidepressants - stop the first SSRI antidepressant gradually (over one week), then substitute another SSRI antidepressant or SNRI antidepressant (Effexor). Fluvoxamine (Luvox), sertraline (Zoloft), or venlafaxine (Effexor) often are effective for Fluoxetine or paroxetine nonresponders (and visa versa).

George Sahadi, of Sahadi's Specialty & Fine Foods, which opened on Atlantic Avenue in 1948, recalls the history of the Atlantic Antic which was founded in 1974, a whopping 33 years ago! "At that time," Sahadi explained, "downtown Brooklyn was just beginning to experience a rebirth and its direction was still iffy."

Affectionately, Sahadi recalled his own family's tradition of opening their store on only one Sunday a year and that was for the Atlantic Antic. He added that the children in his family, like so many other children in the area, always participated in the Antic -- never missing a year until they went off to college.

The Review:

In the wake of September 11, 2001, bioterrorism has become a term with widespread use, though the history of biowarfare dates back to the fourteenth century. Little was then known about how germs caused disease; the stench of corpses was thought to spread infection, so dead animals and plague-infested corpses were catapulted over castle walls as a means of attack.

Bioterrorism and biowarfare have come a long way since then, though the idea is still the same: To immobilize the enemy through disease, whether it be plague or smallpox or viruses that cause hemorrhaging. The modern use of biowarfare agents such as anthrax began with World War I and continues on into the present day.

The UniversityOfHealthCare's Bioterrorism Certificate Program sets out to identify the major agents of bioterrorism, including anthrax, botulinum, and tularemia. Botulinum toxin is the most poisonous toxin known - a single gram could kill one million people if dispersed properly. In a much lower (and safe) dose, botulinum is used under the trade name Botox.

Identifying seven major types of bioterrorism in all, in this thorough manual and CD, each type is then examined in detail. Information is provided about the organism itself, how it is transmitted and how it can be prevented, how it can be diagnosed, and how and why it could be used in bioterrorism. For instance, anthrax is a bioterror threat because it is often fatal in small doses, is odorless and invisible, is easily available, and can travel hundreds of miles before losing potency.

The manual here is provided as a quick reference to important points, while the CD includes graphics, audio, and video that greatly enhance the learning experience, along with offering periodic quiz questions that allow users to test their knowledge. The CD also invites the user go further by taking a graded exam at the end of the section on each disease. If users pass the exam, they will be taken to a page where they can print out their certificate. There is also a cumulative exam, which leads to the general Bioterrorism Certificate.

The language here can be a bit arduous, though a helpful glossary is provided before each chapter to aid the lay person. Timely and thorough, Bioterrorism Certificate Program will open the eyes of those unaware of the deadly threats of bioterrorism and, even more importantly, will educate those that can do something about it.

60 Percent of Physicians Surveyed Fear Dramatic Increase In Patient Wait Time under a Universal Health Care Plan

Physicians polled on their opinions of the healthcare debate favor McCain's plan slightly over Obama or Clinton

In a recent survey commissioned by Jackson & Coker (www. jacksoncoker. com/newsletter/survey-presidential. aspx (http://www. jacksoncoker. com/newsletter/survey-presidential. aspx)), 60% of physicians polled predicted a dramatic wait time for medical procedures as part of any major overhaul of healthcare delivery in the United States.

The survey, entitled "2008 Healthcare Professionals' Opinions on Presidential Health Plans," was sent to thousands of physicians in all major specialties during March, with over 1,000 physicians and health professionals responding.

Participants answered 26 key questions in three major categories: personal knowledge of healthcare reform referred to as "Universal Health Care," their opinions concerning the healthcare proposals of the three presidential candidates, and their thoughts on how healthcare reform can be implemented in the most equitable, cost-effective manner.

"Any health care reform that will prove successful must have the support and cooperation of health care professionals," states Dr. Larry J. Sabato, professor of political science at the University of Virginia and author of A More Perfect Constitution.

"That's why this survey matters. The professionals know the current system inside out, and they are best qualified to evaluate the candidates and their reform proposals, as well as many of the specific reforms that are on the table right now," he adds.

The majority of physician respondents (62%) have been practicing medicine fifteen years or longer. With a seasoned perspective on issues related to healthcare delivery, they voiced strong opinions on the concept of "Universal Health Care" (UHC) and how it should be effectively implemented, as indicated by their unedited comments.

Eighty-nine percent of physicians felt that they are somewhat familiar or very familiar with the concept of Universal Health Care. Interestingly, 46% felt that the current healthcare system--with significant improvements--might be the answer, whereas 38% speculated that a universal healthcare delivery system is called for, however it is designed and implemented.

Who should be covered by universal health care? This critical question, at the heart of the political debate, was phrased as such: "Should universal health care be available to non-citizens in the US?" Six percent of respondents absolutely favored access with no restrictions, whereas 34% opted for access with some restrictions. However, 37% answered "not at all." Certainly politicians and legislators will need to reach some agreement on the matter before any UHC plan is adopted.

Rating the presidential candidates' proposals

The survey focused on another key question, all the more significant in the context of the heated race for the White House. "Which current presidential candidate do you feel would most improve the US healthcare system?" The favorable responses were very close by percentages:

Senator John McCain - 30%
Senator Hillary Clinton - 28%
Senator Barack Obama - 24%.

Interestingly, a significant portion of respondents (21%) did not favor proposals of any of the three remaining contenders. In fact, among former contenders, Ron Paul's proposal garnered the greatest interest.

Overall, half of the respondents felt that the healthcare reform plans of the presidential candidates are extremely important in deciding who gets their vote. This opinion appears to reflect the urgency in addressing apparent shortcomings of the current healthcare model in the United States, as most respondents clearly indicated in their comments.

Dr. Sabato puts the matter in perspective: "There is a near-consensus that the current system simply doesn't work very well, according to this survey. What to do about it is another matter entirely. The three major candidates all have their supporters in approximately equal groupings. Change that is 'substantial but incremental attracts the backing of close to half (46%), though universal health care is backed by 38%--a fact that may surprise some observers."

Implementing Universal Health Care

Regardless of which candidate's health plan is adopted, if any, a wide consensus among respondents (55%) was that any major overhaul of America's healthcare system should achieve a number of important objectives:
Make health care available to the uninsured Reduce costs associated with medical care Streamline all facets of healthcare delivery Make prescription drugs much more affordable.

In what ways could healthcare delivery be more streamlined if UHC were implemented? Four areas were noted most prominently among respondents:
Increased standardization of regulations concerning patient care More efficient processing of electronic medical records Greater access to affordable prescription drugs Doctors spending more quality time with each patient.

The implications for practice management are open to conjecture. Thirty-seven percent of respondents forecasted the number of medical office support will probably increase, whereas 21% predicted that number will stay the same, and 28% believed there will possibly be a decrease.

Patient wait time was also problematic. Along with the 60% of respondents forecasting a dramatic increase in waiting time for medical procedures, 16% felt the wait time might stay the same, and only 4% believed wait time could possibly decrease. More cautious in their assessment, 19% of respondents indicated "It's hard to say at this point."

Furthermore, there was a divergence of opinions concerning the overall benefits of universal health care. A key survey question was "Do you feel that patient care would be better or worse in a UHC system?" Interestingly, 32% said "probably better," but 47% indicated "probably worse." Seven percent felt there would not be any significant change.

Other key concerns:
The survey raised another central question. Physicians were asked to comment on what impact Universal Health Care might have on their careers plans. Certain factors were cited as having an important bearing on career decisions:
Personal income potential - 51% Amount of government regulation - 66% Quality of resultant patient care -- 61% Costs associated with overhauling / streamlining personal medical practice - 42%.

Their "raw" comments included in the survey results clearly establish that, depending on how any proposed plan is adopted, one definite career option for is "early retirement."

The fact that the outcome of the presidential campaign is very much undecided adds an extra layer of uncertainty as to how healthcare practitioners will be impacted by any proposed changes in the current system.

"Even the people who know the system best are not really sure what needs to be done, which candidate can do it best, and whether they can live happily with change. If health care reform was easy, it would have been done a long time ago--and the varying sentiments of these professionals prove it," Dr. Sabato explains

Research published in the Journal of the American Geriatrics Society found that elderly women are more likely to quit smoking than elderly men, while results are just the opposite for studies among younger populations.

Research published in the Journal of the American Geriatrics Society found that elderly women are more likely to quit smoking than elderly men, while results are just the opposite for studies among younger populations.

"Smoking cessation was also observed more frequently among elders who had recently been diagnosed with cancer. In addition, the rate of recidivism (resuming smoking) was only 16 percent among the elderly smokers who quit, whereas previous studies report relapse rates of 35-45 percent," says head researcher Dr. Heather E. Whitson of Duke University Center for Aging. "These findings indicate that older smokers may quit smoking for different reasons than younger smokers."

The study did not directly assess the smokers’ reasons for quitting, but the authors postulate that factors such as lack of transportation, poor financial situation and dementia might contribute to smoking cessation in older smokers. Regardless of reason, the cessation of smoking may lower the risk of death, even when it occurs at an advanced age. The seven-year death rate among non-quitters in the study was 51.6% compared to only 44% among the quitters (although the difference was not statistically significant).

The Association for the Advancement of Retired Persons (AARP) conducted a survey of its members to find that only 39 percent of smokers had been advised by their physicians in the past year to stop smoking. Physicians may assume that older smokers are unlikely to give up one their few remaining pleasures. However, the Duke data suggests that further research is needed to understand the unique motivations and potential benefits of smoking cessation in the elderly.

He's been asked back for the second year in a row and this time, was asked to speak for both full days of the seminar. Obviously he wowed them last year.

On the first day, he'll teach the 200 plus sales professionals the art of "getting in the door." This teaches salespeople how to get past all the screening that some companies put up and make the sale.

As Fasulo puts it, "A Camry is more expensive than a Dodge Neon or a Yugo, but consumers feel that the dependability, great gas mileage, whatever it is, are worth it. People have deemed that the overall value is what's important."

On the second day of the seminar, Fasulo will meet with the inside sales team and teach them the 7 Steps to the Sale. These steps are: Preparation, Introduction, Handling Objections, Discovery, Presentation and Closing. He also teaches "After Close". This is about setting expectations for your customers and putting them at ease, letting them know the steps to be taken after the sale is made. Clear Channel Radio and Comcast have done studies illustrating that after this course is completed, their sales have increased by 17-45%.

Why does Fasulo do it? One reason is that he simply likes to see the look on people's faces after he conducts a seminar and to feel the impact he has made. "I like to help people," relates Fasulo. "I see sales reps who are really struggling, going through trial and error, hoping, hoping, hoping. Then a guy like me comes in and gives them ideas that make a difference. Ottensmeier, MD, PhD, Cancer Research UK Senior Clinical Research Fellow at the University of Southampton, will present interim data from a clinical study of an experimental DNA-based prostate cancer vaccine at the American Association for Cancer Research meeting in San Diego, USA, April 12 - 16, 2008. The data indicates that the combination of this DNA vaccine and electroporation DNA delivery was safe and well-tolerated. Patients treated using electroporation also displayed higher levels of antibody and anti-DOM CD4 responses.

 This academic study is a phase I/II study in 30 HLA A2+ patients with biochemical failure of prostate cancer. The study is testing a DNA fusion vaccine developed at Southampton. The vaccine encodes an immunostimulant sequence from tetanus linked to a sequence from prostate specific membrane antigen (PSMA27). PSMA is an attractive antigen as it is found in the vast majority of prostate cancers and its expression is increased after standard anti-androgen treatment. The study is also evaluating electroporation as a novel delivery strategy for DNA vaccines compared to DNA delivered without electroporation.

The study has completed recruitment of 30 patients and vaccination is ongoing at the third (highest) dose level. Monitoring of antibody responses has been completed for the 20 patients at the first and second dose levels to week 16. Monitoring of CD4 cellular immunity has been completed for the 10 patients at the lowest dose. These 10 patients have additionally been assessed for CD8 T-cell responses. Interim results to date include:

"We are extremely pleased with these observations that our electroporation delivery technology is enhancing the level of antibody and T-cell responses from this novel DNA vaccine against prostate cancer," said Avtar Dhillon, MD, president and CEO of Inovio. "Apart from assessing the safety and tolerability of electroporation, being able to also detect increased levels of immune response is one of the important outcomes that we are aiming to achieve with all of our five current clinical studies for DNA-based immunotherapies and DNA vaccines that are being advanced by our partners."

The development of this DNA vaccine was supported by the UK cancer charities the Leukemia Research Fund and Cancer Research UK, and rights to the vaccine are owned by Cancer Research Technology Limited. The study was supported by Cancer Research UK funding, the Allan Willett Foundation, Inovio Biomedical Corporation, and the Experimental Cancer Medicine Centre in Southampton. The clinical study is a collaborative project between the University of Southampton/Southampton University Hospitals and the Institute of Cancer Research/Royal Marsden Hospital, Sutton, Surrey.

Pyle built HCHP into one of the nation's largest non-profit healthcare organizations. B. Todd Schaeffer, MD a board certified surgeon who specializes in surgery of the nose, face, neck, sinuses and skull base, has been performing endoscopic and laser procedures for over twenty years since graduating from the Mount Sinai School of Medicine. He is the Chief and Associate Chairman of the Department of Otolaryngology - Communicative Disorders at North Shore University Hospital at Manhasset, and holds a dual teaching position as an Assistant Clinical Professor of Otolaryngology-Head & Neck Surgery at The Mount Sinai School of Medicine and Hofstra University School of Medicine North Shore - LIJ Health. Dr. Schaeffer has also been involved in local and national organizational affairs. Locally, as President of the Long Island Society of Otolaryngology-Head and Neck Surgery; nationally, as a committee member in legislative, public relations and carrier relations committees for several different Academies.

Dr. Moshe Ephrat is a fellowship trained and Board Certified surgeon with expertise and interests in facial plastic and reconstructive surgery as well as all aspects of ear, nose and throat surgery. He graduated from Albert Einstein College of Medicine in New York, then completed his residency in Otolaryngology-Head and Neck Surgery at Long Island Jewish Medical Center, of the Albert Einstein College of Medicine. In addition, Dr. Ephrat was awarded a fellowship devoted to facial plastic and reconstructive surgery with Dr. Fred Stucker. He has special interests in rhinoplasty, sinus surgery, and facial injectables using the latest techniques and products.

Dr. Ephrat teaches surgeons in training with his academic appointments as an Assistant Clinical Professor for the Albert Einstein College of Medicine as well as the Hofstra North Shore - LIJ School of Medicine.
