Lyons, MD, introduced the Laparoscopic Supracervical Hysterectomy (LSH) 15 years ago, and now combines this procedure with complex repair of pelvic floor defects, often diagnosed using urodynamics. All are performed using minimally invasive procedures. Myths abound re causes of urine leakage, the main symptom of pelvic floor defects.

“More than 1/3 of women with Stress Urinary Incontinence (SUI) who have given birth do not believe that childbirth increases the risk of involuntary urine leakage,” said Thomas L. Lyons, MD, of the Center for Women’s Care & Reproductive Surgery in Atlanta at www. thomasllyons. com.

“This is a myth. Childbirth and loss of estrogen associated with menopause are main causes of SUI,” he added.

“Regrettably, many women also believe loss of bladder control is a disease, and that it’s a natural consequence of aging. It’s not. But without proper diagnosis using urodynamics testing, women may suffer unnecessarily,” he said.

According to a 2002 landmark study by the National Association for Continence, approximately 14% of American women ages 30-70 suffer from stress urinary incontinence (as opposed to urge incontinence) at a cost of billions of dollars annually. Forty-two percent believe that drinking a lot of liquid puts them at risk, and nearly 1/3 of women age 25-34 with SUI symptoms believe too much sex puts women at risk. These are also myths.

A leading national cause of disability and dependency, stress urinary incontinence is the involuntary loss of urine severe enough to have social or hygienic consequences.

Laparoscopic Pioneer Introduced Minimally-Invasive Pelvic Floor Repairs

More than a decade ago, Dr. Lyons introduced the Laparoscopic Burch Procedure for Stress Urinary Incontinence, and in 1990 he pioneered the Laparoscopic Supracervical Hysterectomy (LSH), which leaves the cervix intact for better pelvic support and sexual function post surgery. Since then he has trained hundreds of surgeons worldwide on the endoscopic procedures he developed. www. thomasllyons. com

However, not everyone is capable of performing these procedures, including LSH. Some surgeons refuse to do laparoscopic procedures on a large uterus. The size of the problem is not an issue for Dr. Lyons.

“It’s especially important that patients choose a surgeon who is experienced in working with lasers and laparoscopy. LSH requires more skill than open abdominal hysterectomy. LSH is easier on the patient, but more challenging for the surgeon,” explained Dr. Lyons.

Endoscopic surgery is a minimally invasive surgical approach that utilizes between three and five small incisions to gain access to internal organs and tissues. Conventional, traditional, open surgery requires a large incision.

SUI: A Common and Treatable Health Problem for Millions

“Urine leakage is the most common symptom of pelvic floor defects, many of which can be fixed surgically using minimally-invasive procedures. Unfortunately, many women experience symptoms for more than six years before being diagnosed,” said Dr. Lyons. “This is a tragedy because of the negative effects leakage has on lifestyle.”

Stress urinary incontinence usually occurs with some form of physical activity, lifting, sneezing, laughing, jogging, bending or stooping. Many sufferers said they abstained from some type of social and physical activity, while others said the condition caused them to refrain from sexual intercourse.

“Despite the emotional and physical costs of this disability, very few women seek treatment for incontinence—often because of their embarrassment,” said Dr. Lyons. Surgical therapy is indicated after conservative therapies have proven ineffective, or if SUI is greatly interfering with a patient's daily activities.

Initial management of the problem can include weight reduction to help lessen intra-abdominal pressure, behavior modification (e. g. changing posture), estrogen replacement therapy in menopausal and postmenopausal women, Kegel exercises, and electrostimulation to strengthen the pelvic floor. Medication to help constrict the muscles in the bladder may also be prescribed.

"Most patients recommended for surgical therapy are wearing incontinence pads due to the severity of their condition, or are candidates for gynecologic surgery for other conditions, such as fibroid tumors, abnormal bleeding or rectocele," said Dr. Lyons. "In those cases, the gynecologic surgeon may recommend that the patient undergo several procedures and pelvic floor reconstruction during the same operation."

Most patients undergoing outpatient endoscopic surgery for SUI are back to work or normal activities within a week--as compared with between five and seven hospital days, and up to eight weeks out of work required with conventional surgery. Other advantages of the endoscopic surgical approach include fewer hospital charges, less blood loss and reduced postoperative pain.

Not All Surgeons Created Equal

Kathy Droze is an active mother of two who experienced urine leakage and was told by both her urologist and GYN that she’d need eight weeks to recuperate after surgical repairs. See video clip www. thomasllyons. com

“I knew their information was totally erroneous,” she said. Even though he wasn’t on her medical plan she opted for Dr. Lyons to perform LSH, complete pelvic floor reconstruction and rectocele repair. “I’d actually have lost much more money if I’d had it done the old way because of time lost from my sales job,” she said.

She advises others not to believe everything they hear on their first or even second medical opinion. Her procedure was done on Wednesday and she was back to work on Monday.

“People don’t do enough investigation. They need to be their own advocates to get beyond the misinformation,” she said.

One thing to be aware of: many surgeons will attempt a laparoscopic procedure and find it necessary to convert to an open surgery with a long incision during the procedure. Make sure to ask your surgeon about his or her conversion ratio. Dr. Lyons’ conversion ratio is less than one percent.

Robert, Jr., Center for Surgical Care and Principal Investigator in the Bioengineering Initiative of the Sheikh Zayed Institute for Pediatric Surgical Innovation. Dr. Peters has a special interest in applying robotics in pediatric surgery, which to date has not benefited fully from these advanced technologies. His research will focus on customized surgical planning and evaluation, enhanced visualization during surgery, and minimal and noninvasive surgical methods. Clinically, Dr. Peters joins Children’s Division of Urology where he will treat infants, children, and adolescents with a variety of urological disorders.

“Robotic surgery has been my passion for the last eight or nine years of my work in minimally invasive surgery in urology,” Dr. Peters said. “The potential exists for robotic technology to improve surgery for children through better visualization, manipulation of tissues, and access to small areas. The Sheikh Zayed Institute provides unparalleled opportunities and resources to lead a paradigm shift in surgery toward greater precision and better outcomes.”

The Sheikh Zayed Institute, which aims to make surgery more precise, less invasive, and pain free, is made possible by a $150 million gift from the Government of Abu Dhabi. The Bioengineering Initiative supports those goals by harnessing the full power of science and technology to make treatment more precise.

“Dr. Peters is an important part of our growing team of surgeons, researchers, and engineers that is coming together to transform children’s surgery,” said Kurt Newman, MD, senior vice president for the Joseph E. Dr. Peters comes to Children’s National Medical Center from the University of Virginia where he was the John E. Cole Professor of Urology and Pediatrics and Chief of the Division of Pediatric Urology at the University of Virginia Health System. Previously, he was at Children’s Hospital in Boston, where his group established the largest pediatric urologic robotics program. Dr. Peters received his medical degree from the Johns Hopkins University School of Medicine.

“I think robotics does need to be seen as a new surgical paradigm,” Dr. Peters said. “It has the potential to enhance our interactions with patients in multiple ways to improve outcomes and reduce morbidity. Men over age 45 are surprised to learn that estrogen, a "female" hormone, is also present in their bodies. Male estrogen is produced in very small quantities, as a by-product of the testosterone conversion process. This process is regulated by an enzyme called aromatase.

Balance vs. Imbalance

Balanced levels of estrogen encourage a healthy libido, improve brain function, protect the heart and strengthen the bones. Estradiol is the most potent estrogen of a group of steroids which includes estrone and estriol. Excessive estradiol levels can lead to the suppression and reduced activity of testosterone. Typically, men's estrogen levels increase with age, and it is not uncommon for a 59 year old man to have more estrogen than a 54 year old woman!

Obesity, pesticides, nutritional deficiencies, prescription medicines and excessive alcohol intake can all raise a man's estrogen levels. When this happens, men frequently lose muscle tone and gain body fat. The end results of high estrogen for men can include obesity, an enlarged prostate, diabetes and a higher incidence of heart disease and cancer.

PSA Levels Not A Good Indicator of Cancer

"PSA level was also found to be not an especially good indicator of prostate cancer. If only PSA levels were used, 82 (17.3%) of the 473 cancers found by TRUS would have remained undetected. Moreover, among men with cancer and a PSA level of less than 4 ng/ml (usually considered a safe level), 42% of the cancers were termed minimal, 42% termed moderate, and 16% were advanced. Serum vs. Saliva Testing

"For reasons that escape rational thinking, conventional medicine persists in using serum tests rather than saliva tests. The results have been disastrous. When using hormone creams or gels, the hormone is absorbed through the skin and into the blood without first passing through the liver. Thus, they are essentially all absorbed in the “free” form. When given orally, they pass first through the liver and 90% of them become protein-bound. For this reason, transdermal dosing is at least 10 times more efficient than oral dosing.

"If one uses serum testing to measure the blood levels achieved by transdermal dosing, the test will fail to measure all the hormone carried by red blood cells. As a consequence, physicians are apt to greatly over-dose their patients.

"When using saliva testing, it is found that the transdermal dose of testosterone when treating someone with testosterone deficiency is only 0.25-0.5 mg in women, and 1-2 mg in men. As the New Yorker article indicated, the transdermal doses of testosterone ranged from 5 mg to 100 mg a day. The same is observed in estrogen replacement therapy – the doses are generally all greatly excessive. The same hormone that brought good health without side effects when in normal endogenous levels will bring on very bad side effects when given in grossly excessive doses.

Conclusion

Testosterone Enhancement or Estradiol Reduction?

Prior to enhancement with testosterone products, reduction of confirmed elevated estradiol levels, via saliva testing, appears to be the more logical approach. Indole-3-Carbinol has shown to be a safe, effective and inexpensive non-prescriptive course of action.

Legendary NFL wide-receiver and three-time Super Bowl champ Jerry Rice scored a touchdown among attendees at Parker Seminars Las Vegas--the world's largest chiropractic seminar--by sharing all the ways he is being a champion for chiropractic. He was joined onstage by fellow chiropractic supporter Linda Cohn, veteran sportscaster and one of the first women to report for ESPN. Both Rice and Cohn are spokespeople for the Foundation for Chiropractic, an organization dedicated to generating positive press for the chiropractic profession, and traveled to Parker Seminars to encourage chiropractors and their assistants to continue delivering natural, quality health care to patients around the globe.

Thinking back to his football career, Rice credits his success on the field largely in part to chiropractic care. "If I had everything in alignment, I knew I could play my best football," said Rice. His training routine included twice-weekly chiropractic adjustments which helped his body recover from all the hard hits and countless tackles. Because Rice firmly believed in the benefits of chiropractic, his teammates began to favor this natural care over the medications that trainers traditionally give. "I wanted to set the standard within football," said Rice. "I knew if chiropractic helped me put up outstanding numbers, it could help my teammates do the same."

And Rice's accomplishments are indeed a testament to chiropractic: he was twice named the NFL Offensive Player of the Year, won three Super Bowls, and still holds nearly every record as a receiver. He retired as the league's career receiving leader. Rice was recently announced as a 2010 NFL Hall of Fame candidate.

The Foundation for Chiropractic Progress (FCP) made its own version of a "Hall of Fame" induction by presenting Rice with a medal for his commitment to chiropractic. "The Foundation for Chiropractic is getting the word out there, and [being a spokesperson] is a way for me to give back," said Rice. "Chiropractic made football great for me. I was at my best and was always at 100 percent."

Joan Wrench is a Bellevue resident. She has taken art classes at Kirkland Arts Center, Pratt Fine Arts Center in Seattle and Bellevue Art Museum, studying painting, drawing, sculpture and printmaking. She has exhibited her work at the Kirkland Arts Center Gallery.

In her latest work, Wrench investigates contemporary themes exploring how we interact with others and how our individual actions affect the greater whole. Using spires and ancient ruins as a departure point, Wrench builds layers in paint and paper collage to create sweeping dramatic fields of color that interact with ghost-like forms. Wrench’s art combines a variety of materials – oil paint, water-based inks, pencil, monotype, collage, etc. – to create distinctly personal work that builds on the traditions of the Northwest Masters.
