Eating a balanced diet with plenty of fruits and vegetables is one way doctors recommend to maintain a healthy lifestyle. But what if every time certain fruits and vegetables are eaten, one develops a sore throat, swollen tongue, itchy mouth, or itchy lips?

Joseph R. Perez, M. D., a board-certified allergist and immunologist in Houston, Texas (www. aachou. com), says that this condition is known as oral allergy syndrome (OAS). "Many people who suffer from seasonal allergies don't even realize that itching of the mouth, tongue, or throat after eating fresh fruits or vegetables can be related to allergies of the nose or lungs", says Dr. Perez.

A new study published in the Journal of Allergy and Clinical Immunology states that this happens in about 25% of people with seasonal allergy problems such as ragweed allergy. The study reports that oral itching during ragweed season may be the first sign that one will develop OAS. Ragweed season typically starts in mid-August and can persist throughout the entire fall season.

OAS occurs because certain allergy proteins are common or similar between plant pollens and certain fruits or vegetables. Dr. Perez says that fresh fruits and vegetables are more likely to cause oral symptoms but the problem can be present even with cooked fruits and vegetables. "People who are sensitive to ragweed allergen may also have a problem eating melons, bananas, cucumber, zucchini, Echinacea, or chamomile tea", notes Dr. Perez.

The danger lies with severe symptoms associated with ingestion of fruits and vegetables. OAS can sometimes be severe enough to cause throat swelling or systemic reactions leading to emergency room visits.

The American Academy of Allergy, Asthma, and Immunology says that patients should see an allergist/immunologist if they:
 Experience itchy mouth from raw fruits or vegetables  Have limited their diet based upon perceived adverse reactions to foods or additives  Have prolonged or severe symptoms of rhinitis  Have nasal polyps  Have co-existing conditions such as asthma or recurrent sinusitis  Have symptoms interfering with quality of life and/or ability to function  Have found medications to be ineffective or have had adverse reactions to medications  Are a child with allergic rhinitis, because immunotherapy may potentially prevent the development of asthma

UCSF Gallo Study Finds Hormone Disorder Drug Could Help Drinkers Stay Sober

A drug prescribed for male and female infertility and menstrual disorders could hold the key to a more effective treatment for alcoholism, according to a study by researchers at the UCSF-affiliated Ernest Gallo Clinic and Research Center.

A drug prescribed for male and female infertility and menstrual disorders could hold the key to a more effective treatment for alcoholism, according to a study by researchers at the UCSF-affiliated Ernest Gallo Clinic and Research Center.

The study showed that "alcoholic" rodents, when injected with the drug cabergoline, decreased their alcohol consumption and alcohol-seeking behavior and were less likely to relapse.

Cabergoline, which is marketed under the trade name Dostinex, is approved by the Food and Drug Administration in pill form to treat conditions caused by excess of the hormone prolactin.

The study, led by Dorit Ron, PhD, a principal investigator at the Gallo Center and associate professor of neurology at UCSF, is now on line (February 20, 2009), in the journal "Biological Psychiatry." (See end of news release for link to paper.)

Notably, cabergoline did not impact the rats' consumption of sucrose and, in a subgroup of binge-drinking mice, the drug did not appear to significantly affect intake of water or saccharin.

"This is encouraging," says Ron, "because it demonstrates that cabergoline is specific for alcohol, but does not affect general reward or pleasure. One of the problems with some existing drugs to treat alcoholism is a side effect that decreases pleasure, making compliance an obstacle to sobriety."

The research builds on an earlier, provocative finding by Ron and her colleagues regarding the protein GDNF (glial cell line-derived neurotrophic factor), which they had injected into rats' VTA (ventral tegmental area) brain region, associated with drug-seeking behavior.

In this earlier study, the scientists had trained rats to consume alcohol. Some, like humans, drank in moderation, while others binged. But when GDNF was administered, both heavy and light drinkers lost at least some of their craving for alcohol. This effect became apparent within 10 minutes and lasted at least 24 hours, the scientists discovered. Importantly, administration of GDNF into the brain prevented the rats from relapsing after a period of abstinence.

While the discovery broke new ground, the scientists knew that GDNF could not be used to treat alcoholic humans because its molecule is too large to cross the blood-brain barrier. So, in the present study, Ron and her colleagues looked at cabergoline, a compound that has been shown in cells to increase the expression of GDNF.

After establishing that cabergoline treatment resulted in an increase of the level of GDNF and activation of the GDNF pathway in the rats' VTA, the researchers sought to test its impact on rodents' drinking habits.

Rats underwent a two-month training program in which they learned to press a lever to obtain alcohol. Researchers found that when rats were injected with cabergoline, they were less likely to press the lever. The higher the dose of cabergoline, the lower the number of lever presses reported. The researchers also found that binge-drinking mice consumed less alcohol after cabergoline administration.

In further study, the researchers found that cabergoline was effective in reducing both craving for alcohol and relapse to drinking. Relapse is a critical issue for alcoholic patients trying to stay abstinent.

As further evidence of the interplay between cabergoline and GDNF, alcohol intake was tested on mice that had been genetically engineered to have a single copy of the GDNF gene, and therefore less GDNF in the brain. As expected, the scientists found that the drinking habits of these genetically modified mice were not affected by cabergoline.

Although the results of the study offer fresh hope to problem drinkers, Ron cautions that human clinical trials are needed before cabergoline can be safely prescribed. Higher doses of cabergoline have been used to treat Parkinson's disease and have been linked to heart valve problems.

"However," notes Ron, "we show that in mice and rats, a low dose of the drug is enough to reduce excessive alcohol consumption, alcohol seeking and relapse. The dose is similar to what is given to humans for the treatment of hyperprolactinemia."

Cabergoline may eventually be prescribed for other addictions. A pilot study conducted on cocaine addicts, cited in Ron's paper, reported a substantial reduction in cocaine use.

In the United States, 17.6 million people -- approximately one in every 12 adults -- abuses alcohol or is alcohol-dependent, according to the National Institutes of Health. But there are just three medications approved to treat alcohol dependence -- disulfiram (Antabuse), naltrexone (Depade, ReVia), and acamprosate (Campral).

Lead author of the study is Sebastien Carnicella, PhD, postdoctoral fellow at the Gallo Center. Co-authors are Dao-Yao He, PhD, senior research scientist; Patricia Janak, PhD, associate professor of neurology at UCSF; Selena Bartlett, PhD, director of the center's preclinical development group; Carsten Nielsen, PhD, associate research scientist; and Somayeh Ahmadiantehrani, graduate student.

Research was funded by the National Institutes of Health and the State of California for medical research on alcohol and substance abuse through UCSF.

On October 21st, 2009, the Los Angeles Brain and Spine Institute enrolled the first two patients on the west coast into the Conscious-3 ( C3) trial, studying the effects of the drug Clozosentan in the prevention of stroke after rupture of a brain aneurysm. The C3 trial is an international phase 3 trial investigating the study drug in preventing stroke due to vasospasm after subarachnoid hemorrhage. Subarachnoid hemorrhage occurs with bleeding in the brain, due to rupture of a brain aneurysm. A brain aneurysm is a bubble that forms on a brain artery, which can be prone to bleeding. While technological advances have occurred in the treatment of brain aneurysms many patients still succumb from the effects of the actual brain hemorrhage. Surgery to fix the aneurysm can prevent further bleeding; however it cannot reverse the effects of bleeding in the brain.

One of the most severe obstacles affecting survivors of brain aneurysm rupture is cerebral vasospasm. This occurs 4 to 10 days after a brain aneurysm ruptures and results in a clamping down, or spasm of the brain arteries. This deprives the brain of blood and oxygen and can result in stroke. 40% of people who survive a ruptured brain aneurysm become disabled or die from vasospasm. A ruptured brain aneurysm itself is very deadly, resulting in death in about half of all cases. To date there has been no effective way to prevent vasospasm.

Clazosentan is being developed by Actelion as an intravenous infusion to treat patients with aneurysm related subarachnoid hemorrhage to prevent cerebral vasospasm and subsequent stroke. Clazosentan shows high specificity for a protein, an endothelin receptor, found in arteries of the brain. In a phase 2 study high dosages of clozosentan were associated with a 65% relative risk reduction in the development of vasospasm.

Patients enrolled in C3 have to have their aneurysm coiled, not clipped. Coiling is a minimally invasive procedure, done by threading a small catheter from the groin to the brain. Once in the aneurysm tiny coils are placed, blocking it from the circulatory system. Clipping is a traditional method of aneurysm surgery, requiring bone to be removed from the skull and the direct application of a clip on the aneurysm. Only specialized centers perform coiling. Dr. George Rappard, medical director of the Los Angeles Brain and Spine Institute, is a Neurointerventional Surgeon specializing in the coiling procedure and the care of critical subarachnoid hemorrhage patients. Dr. Rappard is the primary investigator for the C3 trial in the Los Angeles area.

On the west coast, the C3 trial is also being conducted at Oregon Health Sciences University and Stanford University.

Mavis Cox, an 89-year-old Australian great-grandmother, has shown that age is no barrier to entering the information age. This elderly stateswoman has now started documenting her handicraft experience with her first book entitled "Sewing 101."
