Veterinary neurologist allays fears about cat toxoplasmosis infections for those considering cat adoption during June, which is Adopt-A-Shelter-Cat Month. Pregnancy and cats need not be mutually exclusive with easy solution to eliminate possibility of infection.

Most pet owners have heard that pregnant women should not change cat litter because of a parasitic disease called toxoplasmosis, but many do not know how easy it is to eliminate the possibility of infection, said Dr. Anne Chauvet, a veterinary neurologist (http://www. petneuro. com/about) who founded Veterinary Neuro Services. Simply cleaning the litter box every day virtually eliminates the chance that anyone in the household, including other cats, could become infected, said Chauvet, who has dealt with toxoplasmosis infections in her veterinary neurology practice when the parasite attacks the brain, spinal cord or muscles.

A toxoplasmosis infection is caused by toxoplasma gondii, a microscopic parasite that is transformed into its infective form (sporozoites) in the feces of cats. The parasite sheds microscopic eggs (oocysts) in a cat's feces, and the eggs become infective after two to five days. "That is why it is important to clean the litter daily, before the eggs have a chance to become infective," Chauvet said. "Of course, prospective mothers and those who are pregnant should leave the litter cleaning duties to others during their pregnancies to take no chances."

A patient also can be infected by eating improperly cooked vegetables or if a dog should eat grass infected by outdoor cats. The same goes for humans. If a cow becomes infected from eating contaminated grass, and humans eat beef from that cow, then they can become infected, she said. Cats, however, are the final host because they are the only host that can shed the egg form of the parasite.

Once a cat is infected, the chances for recovery are fair to guarded and recurrence is common. Eating raw or undercooked meat can also cause exposure to the parasite for humans, cats, dogs and other animals. Once ingested, the parasite can morph into one of two forms, an acute form (tachyzoites) or a latent form (bradyzoites.)

The acute form divides rapidly and destroys its host cells until stopped by the immune system. For those with healthy immune systems, symptoms generally are flu-like. In children or those with a weakened immune system, it can invade just about any organ or tissue and can cause just about any clinical sign, such as respiratory infection, muscle or bowel disease and more depending upon where the infection lodges in the body. It also can be fatal if not treated, particularly in those with compromised immune systems, she said.

All of this makes toxoplasmosis difficult to diagnose, Chauvet said. Adding to the difficulty, it may take weeks, or months, before test results show positive for the parasite.

The latent form of the toxoplasma gondii (http://www. petneuro. com/neurologist) parasite divides more slowly, but is more persistent and can stay in the body for years. Research suggests that about 40 percent of cats are positive for the parasite as are about one-third of humans, she said. Not all cats or humans who test positive will develop symptoms.

The polymerase chain reaction test, a type of DNA testing, is the most accurate diagnostic test, particularly when the eyes are affected, Chauvet said. Treatment depends on the part of the body that is affected. In cats, trimethoprim-sulfonamide drugs may be prescribed, but they must be used carefully and for a limited time because they can cause bone marrow suppression resulting in a compromised immune system, she said. Clindamycin and Pyrimethamine are other effective and commonly used drugs, but the later should be supplemented with folic acid as it can cause a deficiency she added.

According to a survey conducted by Hear the World, a global initiative by leading hearing system manufacturer Phonak, exposure to high noise levels was found to not only result in gradual hearing loss, but also stress, aggression or insomnia in 73 percent of those surveyed. In recognition of Better Hearing and Speech Month in May, Hear the World is encouraging all those who are concerned about their hearing health to have their hearing screened this month.

The international survey, which polled 4,405 people between the ages of 14-65 years old in the United States, France, Germany, Switzerland, United Kingdom and Italy, found that close to 50 percent of those surveyed have felt “trapped” by noise in the past. Noise is one of the most common causes of hearing loss. In fact, according to the Better Hearing Institute, 10 million Americans have already suffered irreversible hearing damage from noise and 30 million are exposed to dangerous noise levels each day1 – leading to more than 800 million people worldwide living with hearing loss.

“Outside of a loud occupational setting, the impact of noise on hearing is often underestimated because the damage may take place gradually. As a result, many people do little to prevent the process of hearing loss that takes place throughout their lives due to the noise pollution around them,” said Dr. Craig Kasper, Chief Audiology Officer of Audio Help Hearing Centers. “Better Hearing and Speech Month is the perfect time to take a moment to think about what you are doing to take care of your hearing and to encourage others to do the same.”

Noise Sources and Their Effects

When an individual is exposed at work or at home to harmful sounds – those that are too loud or ongoing – sensitive structures of the inner ear can be damaged, causing noise-induced hearing loss (NIHL). NIHL is characterized by a gradual, progressive loss of high frequency hearing sensitivity over time as a result of exposure to excessive noise levels.

In addition, certain loud noises can also impact us emotionally. Sudden sounds are an urgent wake-up call that alert and activate the stress response – a biological alarm that affects the brain in powerful ways, and often leads to unexpected stress and aggression. Excess noise can cause the production of stress hormones epinephrine, norepinephrine and cortisol continuously circulating in the body, which can lead to problems such as high blood pressure, stroke and heart disease. 2

According to the Hear the World survey, the top five noise sources causing the most annoyance were:

Cassidy, Esquire, of Lubell & Rosen, is lead counsel in a lawsuit filed in Federal Court representing physicians who prescribe pain medications, in the State of Florida, as well as individuals whose ability to obtain medical service is abridged, challenging certain provisions of the law which will limit a physician’s ability to exercise medical decision making.

Although the law was originally intended to combat the problem of over-prescribing pain medication, the statute as written also limits a legitimate and lawful physician’s ability to care for their patients, as a result quality doctors are subjected to penalties and sanctions for their current practices. The state has also refused to fund the single most effective means of combating abuses in the system by choosing to not implement a drug monitoring program, instead opting for regulations which create virtual chaos for doctors and patients alike.

In the lawsuit, the plaintiffs maintain the law is discriminatory due to its restrictions which bars patients from receiving more than a three-day supply of drugs if they do not have insurance. They also claim it violates freedom of speech rights by limiting clinic advertising. Other claims are that various provisions are unconstitutionally vague, which could result in doctors being arrested or exposed to administrative penalties for violating the law without having known they were covered.

Minnesota’s small towns and rural areas will take center stage early this June to show how they are “Working Better Together for the Common Good” – the theme for the combined Symposium on Small Towns and Minnesota Rural Summit, June 6 & 7 at the University of Minnesota, Morris.

It’s an opportune event as budget deficits and changing priorities pull more and more federal and state resources out of rural areas and leave growing responsibilities for all kinds of community and economic issues to local leadership. Relative to a metropolitan area, the per-capita leadership requirements of small towns are staggering. How are our small towns and rural areas innovating to overcome this challenge?

This question will be the focus of combined Symposium & Rural Summit, which itself is a model of the cooperation happening across rural areas. This will be the fourth annual Symposium on Small Towns sponsored and held at the University of Minnesota, Morris, and the ninth annual Rural Summit held in a different Minnesota location each year. It is the first time these events have combined. The event will showcase actions underway by people who recognize the changing demographics and landscape of rural Minnesota, the evolving economic opportunities, and who work together to sustain their communities. Organizers are also planning a legislative forum and are inviting the gubernatorial candidates to share their ideas.

“It was time to bring all the voices together,” said Jane Leonard, President of Minnesota Rural Partners and a co-host of the combined Symposium-Summit. “It’s clear that rural America is getting shut out of most policy decisions, especially within the federal arena, unless there is a compelling economic interest, such as renewable energy. It’s now visibly apparent how much of the human infrastructure of state and federal government has disappeared and how adversely this can affect small towns. For example, state and federal staff that provided technical assistance and coordinated funding help to small businesses used to be more widely available. Now they are highly centralized in regional centers or worse yet on the federal level, outside of Minnesota and the Midwest altogether. We are disinvesting in people, places and good ideas at exactly the wrong time. The most vulnerable parts of rural America are paying the price.”

Sharing resources to reduce costs is not new, especially in rural communities. Counties have worked on joint-powers agreements, higher education facilities have partnered with cities, and community groups have worked together with state agencies. But often the sharing is within distinct sectors: housing, health care, education, business. There’s a compelling need to work together across sectors, to renew the underlying social contracts of a healthy democracy, those that set aside more narrow self-interest and look at the big picture, the common good. And common good investments can also be cost-effective and cost-efficient.

Despite federal and state cutbacks, Minnesota is still resource-rich compared to other states. It’s not always a lack of resources but a lack of coordination across sectors. “Coordination was something the federal and state governments used to support, with very little monetary investment and a lot of return,” said Leonard, whose non-profit group, Minnesota Rural Partners, used to be funded by federal dollars to do just that. “The coordinating money no longer exists. But we continue to do what we can, because we believe in the ideals of common good.”

Common good has been an admired value in Minnesota’s small towns, but small towns are far from “common.” Despite the traditional stereotypes, “there are many ‘rurals’ in Minnesota,” said Ben Winchester, data analysis coordinator for the Center for Small Towns and the Symposium coordinator. “There are rural metropolitan, recreational and the remaining manufacturing/agricultural-dependent areas of the state.” In this diverse landscape, people and groups are witnessing shared planning requirements, growth patterns and decision-making needs. Today, some Minnesota small towns are challenged by the increasing complexities of growth—housing and development, tourism and environmental protections, and the influx of immigrant populations. Others need “smart decline” strategies as the population leaves.

Co-hosting the Rural Summit with the Symposium brings the annual summit for the first time ever to western Minnesota, said Leonard, who has worked in rural development for more than 20 years. “The Morris area is a model.” She noted. “It’s demonstrating the promise of 21st-century rural life: renewable energy; value-added agriculture; small, diverse business development; higher education; and creativity through arts and culture. It’s building a rural vision of Richard Florida’s (author of Rise of the Creative Class) new economic development maxim: technology, tolerance and talent.”

The symposium and summit will identify programs and services that can help communities work better together. Among the sessions will be one which Winchester has dubbed, “speed dating”–an opportunity to provide participants with quick one-on-one networking with resource providers.
