Research in the Annals of Internal Medicine finds that not all patients with back pain get better after spinal manipulation. This research produced a ‘prediction rule’ which reveals factors in patients with back pain that predict if they would or wouldn’t get better with physical therapy treatment.

Patients were divided into two groups. The first group received ‘high-velocity thrust spinal manipulation’ by their physical therapist plus therapist-guided exercises. The second group got only exercises. Each group saw their physical therapist five (5) times. Six months later, the patients that got both manipulation and therapist-guided exercises needed less medicine, less health care, and had missed less work.

According to Kelli Goedde, PT, DPT, OCS, physical therapist at ProRehab in Evansville, Indiana, “The study found that if the pain was less than 16 days old and did not extend below the knee, 9 out of 10 patients reduced their back pain by 50% after one manipulation. I am seeing very similar results with my patients at ProRehab.”

For medical research to be regarded as best practice, a study has to be repeated in different places. Since the study in Annals of Internal Medicine, this prediction rule for using manipulation with back pain has been confirmed in three other medical journals.* More research found that patients who did not receive thrust manipulation were eight times more likely for back pain to worsen. The prediction rule found two important factors for success. One was back pain less than 16 days old. The other is that pain does not extend below the knee.

In 2004, the FDA sponsored a study on occurrences of rhabdomyolysis, a sometimes deadly side effect of cholesterol-lowering statins, generally believed to be rare after the withdrawal of cerivastatin. The FDA study looked at patients using statins for a period of one year, with results showing that only 0.44 in 10,000 patients will develop this sometimes life-threatening condition.

"The FDA study may show that rhabdomyolysis is a rare condition in overall statin patients," says Johnson Chen, Co-Founder of eHealthMe. com. "However, what that study doesn't do is look specifically at the risk of the condition in the many patients already known to be susceptible to statin side effects. We analyzed a more extensive data set concentrating on the frequency of rhabdomyolysis in only those patients who had been known to experience side effects from statins usage, in an effort to provide better information to help those patients most at-risk make better decisions in their choice of healthcare."

Findings:
Side effect occurrences varied greatly between individual statins. Of seven statins analyzed, rates of rhabdomyolysis and myopathy accounted for 3.29% to 7.76% of all side effects in patients who experienced one or more adverse reactions. Evaluation of the adverse reports indirectly suggests the rate of rhabdomyolysis may be significantly higher than the previous FDA study estimated, because many statin patients are elderly or have impaired kidney function, thus being more likely to experience the statin side effects.

Suggestions:
New kinds of information about statins should be made available to patients and professionals to aid them in making healthcare decisions. Wilderness Therapy Programs Ramping Up for Surge in Holiday Admissions

Between school vacations, family gatherings, and an often stressful season, more families are turning to wilderness therapy programs for support in helping their troubled teens during the winter holidays.

The holiday season is a time for sharing, festivities, and good cheer. But for families with troubled teens, the holidays can be one of the most anxiety-ridden times of the year. As a result, parents of adolescents who are barely getting by in school or at home are taking advantage of the upcoming holiday break to enroll their teens in wilderness therapy.

Like other vacation periods such as summer and spring break, winter vacation means more down time, which leads to less rules and structure and more opportunities for struggling teens to engage in delinquent behavior. "Young people tend to focus more on the social scene during school vacations," says Kathy Rex, Executive Director of SUWS Adolescent and Youth Programs. "Winter, even more so than summer, brings out school parties, which often come with underage drinking, substance use, and heightened emotions."

More down time also means more family time, which can escalate interpersonal conflicts and make evident problem behaviors that may have been less visible when school and work were in full swing, explains Michael Ervin, Regional Director at SUWS of the Carolinas, a therapeutic wilderness program in North Carolina.

The holidays can be full of stress and anxiety for adults as well as teens. Add to the bustle of family gatherings a teen who is angry, defiant, depressed, or otherwise acting out, and many parents are pushed to their breaking point.

"When the family system is stretched beyond its means, the holidays are a good time for everyone to go to their own corners, regroup, and then reconnect," says Rex. "With guidance from the field staff and therapists, the wilderness experience can help the family get their relationships back to a pure, enjoyable place where they recognize each other's strengths and abilities and have meaningful, productive communications. It's a strange irony, but being alone often helps us appreciate each other more."

Short-term wilderness programs keep struggling teens healthy, happy, and out of trouble, with as little time away from school as possible, notes Rex. The average wilderness program lasts 30-45 days, but programs like SUWS offer stays as brief as 14, 21, and 28 days.

"Time in the wilderness is an opportunity for at-risk adolescents to identify problems and focus on what is truly important in their lives," says Sue Crowell, Senior Vice President of Outdoor Programs at Aspen Education Group, the nation's largest and most comprehensive network of therapeutic schools and programs. "The teens may be missing out on this year's family functions, but a few weeks in the wilderness can change the whole next year around, and the years after that."

Spending the holidays in the wilderness can be one of the most memorable, enriching experiences a teen can have. Many wilderness programs make the holidays a special time in the field, honoring different religious holidays, offering days of reflection for students of all faiths, and preparing special meals on Thanksgiving and other holidays.

Even though the teens aren't at home with their parents and relatives, there is still a strong emphasis on family. Students of wilderness therapy become part of a cohesive unit of peers, or "wilderness family," that helps them understand their place in their own family and the importance of those family bonds.

According to Mike Bednarz, MA, MBA, Executive Director at SageWalk the Wilderness School in Oregon, many staff members volunteer to work on the holidays because it is a particularly rewarding time to be part of a child's experience away from home. "My times in the field during the holidays have been some of the best experiences I've had," says Bednarz. "Students are more attuned to wishing they were home, more thankful for what they have, and more willing to take an honest look at their lives. They may not be at home, but they know how much they are cared for."

The holidays can be an especially therapeutic time to be in the wilderness. Students often share favorite holiday memories and family traditions, and make special cards and gifts to send home. "Spending the holidays in the wilderness can be a real learning opportunity. It's a good time to reflect on what is missing in your life, what the holidays were like last year, and what they could be like next year," says Bednarz.

Wilderness therapy is an opportunity to address real concerns that tie into the teen's long-term health and safety in a caring, nurturing environment. And while distance is never easy, especially around the holidays, says Rex, "one holiday apart can bring a lifetime together."

The interplay between baseball and politics goes way back, and 100 years ago saw the election of the first former big league star to high elective office, beyond the likes of small-town mayor. The star was Cap Anson, who held Chicago's third-highest post, city clerk, from 1905 to 1907.

Politics and baseball clashed recently in the halls of the U. S. Congress, where lawmakers looked into steroid use by major league players. The interplay between baseball and politics goes way back, and 100 years ago saw the election of the first former big league star to high elective office, beyond the likes of small-town mayor. The star was Cap Anson, who held Chicago's third-highest post, city clerk, from 1905 to 1907.

After a colorful career on the diamond, Anson, the lone player to reach 3,000 hits before 1900, was elected on April 4, 1905, and served two years. A 1906 bid to be elected sheriff failed miserably, as he lost in the primary. His city clerk tenure, by all accounts, had been lackluster at best. For example, in an editorial, the Chicago Tribune said, ``When an elective officialÂ’s eagerness for some other elective office is of the sort that makes him a coward in his present office that cowardice is not a good qualification for another job. The stories which Capt. Anson told on the platform during his campaign a year ago seemed then to win him votes, but the people may not be so `easyÂ’ a second time. They may turn the light on the captainÂ’s record. While the captain is in his present office he should try to be a real city clerk.Â’Â’

Anson's colorful persona had shined while leading the Chicago White Stockings/Colts of the National League from 1879 to 1897. He was often gruff in commanding his players, a far cry from the new touchy-feely style of modern-day chieftains. For example, new Washington Nationals manager Frank Robinson recently said, "I knew that, coming back [to manage], I couldn't be the same type of manager I was when I left." For one thing, "You have to listen first. You can't just tell people: 'Because I'm the manager, that's why.' You have to explain things to them. You have to listen to them. You have to, sometimes, adjust, and say, 'Okay, you're right. Maybe we'll try it that way.'"

The Washington Post's Barry Svrluga, who reported the above comments, added, "Take infield practice. When Robinson played, the regulars took infield every day. He still feels that's the best way to go about things, that it loosens you up, gets you focused. But during his time in Montreal [2002 to 2004, after not managing for more than a decade], the players asked him whether it was necessary. They do so much work to prepare, they said, that infield every day could tire them out. "I said, 'Okay, we'll try it,'" Robinson said. "In the past, I'd have said, 'No. We're doing it each day.'"

Cap Anson 3 closely surveys tricky and dirty play over the first quarter-century of the National League, 1876 to 1900, in addition to being the definitive biography of McGraw through 1900, covering about 85 percent of his playing career. For their on-the-field play during that period, the book gives special focus to Baltimore of the 1890s (McGraw was its third baseman) and Chicago of the 1880s and 1890s (Anson was its captain-manager).

Today, the 1890s Baltimore Orioles are remembered for having toyed with the playing rules, which used to be more liberal and more liberally enforced. For one, McGraw was especially skilled at fouling off pitches, in an age before batters incurred strikes for doing so. The 1880s and 1890s Chicago teams are hardly thought of in a tricky or dirty light; they are mostly cast as a study in contrasts: Chicago won several pennants through 1886 and then none into the early 20th century. However, Chicago players did lots of tricky stuff too. One of Anson's notable tricks was doing what Barry Bonds (when he's healthy) cannot: when being intentionally walked, jump from one batter's box to the other, to try to hit the ball. Sometimes in doing so, Anson succeeded in making hits.

Also featured in the book is Tommy Tucker, a first baseman who played mostly for the Boston Beaneaters. Tucker was arguably the 19th century's dirtiest player, especially because he liked to block runners on pickoff tries. Also studied closely is the singular Mike Kelly; Cap Anson 2: The Theatrical and Kingly Mike Kelly: U. S. Team Sport's First Media Sensation and Baseball's Original Casey at the Bat (2004), Kelly's otherwise-definitive bio, omitted the vast majority of his trickery. Instead, Cap Anson 2 focused on Kelly's personality, including by arguing that he was the most likely model, if any single player was, for the baseball situations of Casey in the 1888 poem "Casey at the Bat."

Kelly, by the way, was the first baseball player to be paid a lot of money to perform a bit part on stage (in 1888), while Anson was the first to receive star billing (in 1895). Author Rosenberg's first book, Cap Anson 1: When Captaining a Team Meant Something: Leadership in Baseball's Early Years (2003), featured the more active roles that players used to take in leading teammates. Into the 20th century, a team's captain, not its manager, was in charge of arguing with the umpire.

A future book in the Anson series will be his full-blown biography. It will contain detail about his city clerk tenure, which the author can make available to media with an immediate interest in the subject.

Noteworthy about his presence on the Chicago Democratic slate in 1905 is that he added geographic balance, as he was the only one of its top four candidates to be a resident of the South Side. A likely help to his electoral image was his reputation for rugged honesty. As a player, he had regularly bet on baseball, but only on himself or his team to do well, and did not associate with gamblers. In recent years, Pete Rose's association with gamblers, while betting on baseball as Cincinnati's manager, was arguably what most justifies his current ineligibility for election to the National Baseball Hall of Fame.

Anson and John McGraw, perhaps best known for his 20th-century managing of the New York Giants, had a lot in common, including in playing billiards (they enjoyed each other's company) and betting. However, while Anson was squeaky clean, at least when not bending the playing rules during his big league career, McGraw, a main subject of a pair of new books (the other is Frank Deford's enjoyable The Old Ball Game: How John McGraw, Christy Mathewson, and the New York Giants Created Modern Baseball), maintained a close business relationship with well-known gamblers including Arnold Rothstein, the inspiration for the throwing of the 1919 World Series.

Beyond Cap Anson 3's totally new take on trickery and dirty play in baseball's first decades, the book contains one major piece of news: Its second appendix refutes at least 66 years of prevailing thought that McGraw and Wilbert Robinson, co-owners of a Baltimore bowling alley, were responsible for the origin of the sport of duckpins. Cap Anson 3 traces quackery on the subject as far back as future legendary writer Shirley Povich in 1939. Instead of finding support for McGraw and Robinson as originators, Cap Anson 3 cites an article on the subject that precedes their going into business together.

Mr. Finkel manages ICF ConsultingÂ’s on-site technical assistance to the U. S. Missile Defense AgencyÂ’s (MDA) Facilities, Siting, and Environmental Management Division, where ICF Consulting supports the development of Environment, Safety, and Occupational Health (ESOH) policies and guidance, as well as the implementation of MDAÂ’s environmental management system for developing and executing the MDA ESOH compliance program.

In past positions at ICF Consulting, Mr. Finkel played a key role in leading ICF ConsultingÂ’s projects at the U. S. Environmental Protection Agency, and was also instrumental in expanding ICF ConsultingÂ’s international work in advising on cleaner production and waste management business in Asia, Europe, and Latin America.

Georgia Dentist Uncovers Obscure Law That Helps Dental Patients With Severe Anxiety

Dr. David Kurtzman has discovered an obscure Georgia Law that forces insurance companies to cover Sedation Dentistry for Georgia Dental Patients with severe anxiety and other disorders. Dr. Kurtzman has been practicing Dentistry in Georgia for over 20 years and he just found out about the law so the general public knows little or nothing about "Senate Bill 66".

Severe anxiety, phobia, severe gagging, life threatening allergies and the inability to use local anesthetics are among the myriad of reasons that over 100,000 Georgians are unable to receive the dental care they need in a dentists’ office. For these people, dentistry ranges from almost torture to a life threatening experience – yet many want and need care.

In 1999, to little fanfare, the state senate of Georgia passed a law to help these people. Since their only alternative is to have their dental work done while they are truly and fully asleep, “Senate Bill 66” mandates Medical Insurance to pay the additional costs of general anesthesia and hospital costs for these people. Once the often prohibitive costs of being fully asleep in the hospital are handled, a lot more of these people can afford the dental care they really need.

Dr. David Kurtzman, a Marietta, Georgia, dentist who has been treating these cases for over twenty (20) years says, "No one really talks about this law – certainly not the insurance companies!” He had worked for years trying to get these costs paid. Even when he got to speak with people within the insurance companies he called, nobody ever mentioned it. Not until a chance call to the Insurance Commissioners’ office in Atlanta lead Kurtzman to the obscure bill did his office start getting more and more coverage for their patients.

The law states that medical insurance must pay hospital and anesthesia fees for any person for whom a successful result cannot be expected using local anesthesia (Novocaine, etc.) “Because of physical, intellectual or other compromising medical condition” of the insured patient. Anyone who really needs it can now expect coverage for sedation dentistry.

“We are seeing people who have lived with pain and infection for literally years!” says Kurtzman. Care under general anesthesia in the hospital operating room gives thousands of these people hope for a healthy, painless and beautiful smile.
