November 14th 2003 marks the fourteenth annual World Diabetes Day. Diabetes is a chronic condition that affects people of all ages in all areas of the world. Inadequate care can lead to serious health complications such as blindness, kidney failure, neuropathy (degeneration of nerves and nervous system), amputation, heart attacks and death.

Type 1 diabetes is caused by a process which destroys insulin producing cells in the pancreas. Insulin is vital for the survival of patients suffering from Type 1 diabetes and must be administered daily throughout the life of the patient by injection. The International Diabetes Federation estimates that there are 5.3 million people world-wide who suffer from Type 1 diabetes, and thus require insulin every day.

The first patient to be treated with insulin was a Canadian child by the name of Leonard Thompson in 1922. Over 80 years after the first patient received this life saving treatment many people with diabetes in the developing world still have difficulties accessing insulin. This leads to a life expectancy which can be as low as 12 months for a child in rural sub-Saharan Africa compared to over 50-60 years for a child in Europe.

The stories below highlight the contrasting challenges that two children face:

My name is Mary and I live in the capital city of Zambia. I am 15 years old now. When I was 11 years old, I was admitted to a local health centre and was diagnosed as having malaria. After a few days I wasnÂ’t getting any better so I was sent to the main teaching hospital. After a long time there a doctor diagnosed diabetes.

My family has an income of about £91 per month. Sometimes my family need to spend as much as £42 if I have to go into hospital.

I now attend a free diabetes clinic held once a week at the main teaching hospital. I tend to go about once a month for a check up and to get my insulin. They also test my blood sugar, as I do not have a machine or testing strips at home as these are too expensive. When I go I need to miss school as the clinic takes place during school hours and it takes me over an hour to get there. I need to take a bus there. It costs about £1.00 to get from my home to the clinic and back. Sometimes when money is a problem I can’t go to the clinic. But I am lucky living in the capital. If I lived in the rural areas I might have to travel up to 100km to a hospital and I would not be able to afford this more than once a year. And I know that these hospitals are often out of insulin.

I was able to attend a Youth Camp organised by the Diabetes Association in my country. I learnt a lot about my disease, how to inject myself, the foods I should and shouldnÂ’t eat. And also had a chance to meet other children with the same condition as me.

I am lucky to get my insulin for free at the pharmacy, but sometimes it is out of stock so then I have to buy it at a private pharmacy and this can cost as much as £12 for a bottle – which lasts about a month. The hospital pharmacy hardly ever has any special insulin syringes so if I am lucky I get them free from nurses and doctors working in the intensive care unit. Most of the time though I have to pay £1.40 for a month’s supply of syringes. Even though these are meant to be used just once, I use them for a week or two depending on how much money my family has. My family does not have a fridge so my insulin is kept in a clay pot with water under my bed.

The hardest part of my treatment is injecting myself every day and not being able to enjoy the same foods as my family and friends. I worry if we donÂ’t have enough money for insulin. We are 8 at home and a lot of money is spent on my care and they also worry about me too much.

My name is Tom and I am 14. I have had diabetes for 2 years now and live on the outskirts of London. My GP referred me to the Hospital after I was found to be drinking a lot, passing a lot of urine and losing weight. He did a blood test in the surgery and it was then that I found out I had diabetes.

Every 3 months I attend a consultation with a diabetes specialist nurse and we go through any problems I have. If ever I have any problems I can phone her and get an appointment or she can sort things out on the phone. The nurse has also spoken to my teacher and the school nurse so they understand my diabetes and what I need at school. I am lucky as the NHS provides all my care for free and the Hospital is just a bus ride away or my Mum takes me there by car.

Last year I went to the Diabetes Care camp and had a great time doing lots of adventure stuff with other kids with diabetes. I really learnt how to monitor my blood and change my insulin.

I get my insulin and pen for injections for free. I also have a machine for testing my blood sugar so I can change the amount of insulin I need if I have a big meal or do sports.

Besides remembering to do regular blood tests, not being able to eat all the foods I like all the time and that my mum worries about me too much, I lead a normal life.

Orthopedic surgeons in Singapore (http://www. itravelmed. com/search. php) are now treating shoulder injuries with a homegrown technique that has proven much more successful compared to the current gold standard.

Called the Triple Endobutton Technique, this relatively simple yet highly effective concept was developed as a response to the high failure rates associated with conventional methods of mending Acromioclavicular Joint (ACJ) dislocations (http://www. singaporemedicine. com/healthcaredest/sg. asp).

"Injuries in and around the shoulder are common in today's athletic society. ACJ injuries are often sustained by cyclists, as well as those who engage in contact sports such as rugby, martial arts and American football," says Dr Lim Yeow Wai, Specialist for Orthopedic Surgery at Raffles Hospital (http://www. singaporemedicine. com/hcp/hcp1.asp).

Dr Lim developed the new technique two years ago, after observing that conventional procedures do not offer lasting solutions to those who have fractured or dislocated their ACJ.

"The shoulder is a very complex and unstable joint in the body which can get easily injured following an awkward fall. Many types of surgeries have been developed in the past to repair the bone and torn ligaments, but so far, outcomes from these methods are far from satisfactory," he explained.

The acromioclavicular joint is located at the top of the shoulder. It is made up of two bones - the clavicle and the acromion - and two ligaments which gives the joint its stability. Depending on the severity of the injury, a person may sprain or tear one or all of the ligaments. If not properly treated, ACJ injuries can lead to significant loss of strength in the affected limb. It may also cause arthritis of the joint.

"Presently, most of the treatment methods employ rigid forms of fixation using metal implants such as hook plates, pins and screws. This rigidly secures the two bones, but often leads to failure because the ACJ joint is not meant to be fixed in place. It is a flexible joint which can rise and rotate during movement," said Dr Lim.

Furthermore, screws and pins are often ineffective because they usually work themselves loose after some time, due to necessary movements in and around the shoulder area.

The Triple Endobutton technique (http://www. singaporemedicine. com/leadingmedhub/leadingmedhub1.asp), on the other hand, is non-rigid and allows for normal movement of the joint. All that's needed is three tiny buttons and two strands of Fibrewire suture. The buttons are made of stainless steel and are only about 1cm across.

During the operation, the suture is looped through the button holes and over the shoulder bone, resulting in a 'snow shoe' hold that allows for fluid movements while stabilizing the joint.

According to Dr Lim, this method gives even greater strength compared to the natural ligaments.

The entire surgery takes just 45 minutes under general anesthesia with the patient in a beach chair position. So far, Dr Lim has performed 11 such operations with only one failure, giving this technique a failure rate of less than 10 percent. This is less than half the failure rate of ACJ surgeries using conventional methods.

Post-operation, a sling will be applied for about four weeks. Patients can generally engage in light duties after that.

"Normal activities and household chores will not pose a problem at all, although we advise patients not to go back to professional contact sports till a few months later," said Dr Lim, who is himself a martial arts exponent.

The best part is that these buttons can safely stay in the body for life.

"The buttons do not rust and it is unlikely to cause infection, too, so there will be no need for a second operation to remove them once the shoulder has healed," said Dr Lim.

Currently, the Triple Endobutton technique is only performed in Singapore.

The cost of the Triple Endobutton surgery (http://www. singaporemedicine. com/intl_pts_guide/preparation. asp) is US$6,500. This compares favorably to conventional surgeries in the US, which usually range between US$10,000 and US$15,000.

Dr. Robb says most program directors at veterinary technician schools agree that all states should require veterinary technicians to be licensed. The question then becomes what is the job description of a veterinary technician? Currently veterinary technicians are laboratory techs, radiology techs, surgery techs, dental hygienists, anesthesia techs as well as client educators. They do all these things but in many states are not required to be licensed. With so much knowledge and expertise required of them, it's no surprise why there are so many tragic pet injuries and deaths.

Dr. Robb is calling on fellow veterinarians for support. “I believe the main reason for our current situation is veterinarians fear that a licensed technician would demand a greater salary and thus upset the financial viability of their clinic or hospital. But underpaid technicians soon move on, perpetuating the cycle of training from scratch that is equally or more costly. I proved in my own practice that having a highly-skilled staff and compensating them accordingly only helps the finances of the practice.”

Federal health officials are examining rare reports of blindness among some men using the impotence drug Viagra. FDA clinical trials show that taking Viagra, especially at high doses, can cause some retinal dysfunction and affect sight for a number of hours. Studies show visual disturbances described as a bluish color tinge to the visual field and light sensitivity.

The Food and Drug Administration still is investigating, but has no evidence to determined the long-term effects of Viagra yet that the drug is to blame, said spokeswoman Susan Cruzan.

Viagra has also been in the news because of its possible link with sudden failure of the blood supply to the optic nerve. That effect eyes & causes blindness called NAION, or non-arteritic anterior ischemic optic neuropathy. It can occur in men who are diabetic or have heart disease, the same conditions that can cause impotence and thus lead to Viagra use. The FDA has 50 reports of the blindness. Viagra has been taken by more than 23 million men worldwide.

What is NAION?

NAION is one of the most common causes of sudden vision loss in older Americans. The condition leads to swelling around the optic disc, a connection point where the optic nerves reach the back of the eye. Ultimately, this swelling compresses the optic nerves and causes a rapid drop in vision. About 6,000 people develop NAION each year, according to the National Eye Institute.

How does NAION develop?

The symptoms are painless and often appear without notice. In a study led by researchers at the University of Iowa, about three quarters of NAION patients first discovered their vision was blurry and impaired after waking up in the morning. The condition starts in one eye and can cause complete blindness if both eyes are eventually affected.

Is there a strong reason to suspect that Viagra raises the risk of blindness?

The seven men in Pomeranz's recent study all developed NAION soon after they took Viagra. After taking two blue Viagra pills the day before, one 50-year old man said he felt "a flashbulb go off in my eyes." The onset of vision loss within hours of taking Viagra "supports an association between use of this agent and NAION," Pomeranz wrote. However, NAION appears suddenly in some 6,000 people a year for a variety of unknown reasons. "We don't really know what the natural history of NAION is," said Dickersin. Pomeranz is also cautious about Viagra causing NAION: "A definite casual relationship cannot be established at this time," he noted.

Is the vision loss substantial?

NAION can be disabling, especially if it affects both eyes. In the National Eye Institute study, whatever gains the patients had were partly lost after two years. But even with declining eyesight, most had better vision than when they were first afflicted with NAION. "These improvements are a noticeable difference," Dickersin said.

Is there any treatment for NAION?

There is no effective treatment as of yet, although researchers are examining different therapies in clinical trials.

Who is at risk for NAION?

Those who have bad eyesight already appear to have the highest chance for developing NAION. While people who have diabetes, hypertension or high cholesterol seem to be a greater risk for NAION as well, there is some debate on how much. "None of these systematic disorders are firmly associated with NAION, and diabetes could only account for only a minority of the cases," stated Simmons Lessell, MD, of the Massachusetts Eye and Ear Infirmary in an editorial for the Archives of Ophthalmology.

CASE STUDY

In the study, seven patients between the ages of 50 and 69 had typical features of NAION within 36 hours of taking Viagra. Seven similar cases have been previously reported. All of the patients, though, had at least one arteriosclerotic risk factor, including hypertension, diabetes, hypercholesterolemia, or hyperlipidemia, and they also had a low cup-to-disk ratio, which is a way doctors measure the small circular indentation where the optic nerve connects to the eyeball. Pomeranz says the low cup-to-disk ratio means that the blood vessels and nerves are tightly bundled together into the small space in the back of the eye. And it is this constriction of blood vessels, he says, which promotes NAION.

What are the general precautions for taking Viagra?

--Men who are taking medications that contain nitrates, such as nitroglycerin, shouldNOT use Viagra. The two medications taken togethermay decrease blood pressure to the extent that adverse effects may occur.

--Viagra should NOT be used by women or children.

--Viagra's use in combination with other ED treatments has not been studied, therefore, its use in combination with other treatments is NOT recommended.

Virginia Adams, PhD, RN, a leading nurse educator and consultant to the National League for Nursing on global affairs and diversity, has been tapped to chair the Steering Committee of the International Council of Nurses Education Network (ICN-EN), putting the NLN in the forefront of planning the network's agenda and maximizing its impact across borders worldwide. Earlier this year, Dr. Adams served as the NLN liaison to the International Nursing Education Services and Accreditation (INESA). This joint task force of the NLN and NLN Accrediting Commission played a key role in the establishment of the ICN-EN. The purpose of the education network is to provide a forum to address nursing educational issues worldwide.

"I am thrilled to have Dr. Adams at the helm of this evolving area within nursing education," said NLN CEO Beverly Malone, PhD, RN, FAAN. Chief among these is a new ICN-EN interactive website, designed to facilitate communication on issues pertinent to nursing education and standards, and health care policy and delivery. The website will also feature a comprehensive calendar of nursing education conferences around the globe; and serve as a go-to online resource for the nursing education community. At the Steering Committee's first meeting, an October 20 teleconference, it was determined that the web-based discussion forum will initially focus on two high-priority issues: the shortage of nurse educators and primary care health care. The committee will collect, and post on the website, best practices from within their regions. In addition, questions about how nursing education programs are incorporating primary health care into their curriculum will be posted for discussion; given global variations in language and understanding, careful attention will be paid to clarifying and defining terminology. ICN-EN's inaugural e-newsletter, another Steering Committee project, is set for publication in early 2010; it will include bio-sketches of the international Steering Committee.

Under Dr. Adams' direction, the Steering Committee will also draft a governance structure for the network and seek ways to partner with other groups within ICN, such as the ICN Student Nurses Network. Looking ahead to 2011, the Steering Committee plans to deliver its first report to the ICN during the group's biennial conference in Malta.

A professor and special assistant to the vice provost at University of North Carolina-Wilmington, Dr. Adams has worked to broaden the scope of international initiatives in nursing education; university programs were expanded or created in countries around the world including Barbados, England, Japan, and Peru. As a fellow in the WK Kellogg Foundation Community Partnerships program, Dr. Adams traveled around the US and abroad learning about delivery of health care services to underserved populations across racial and ethnic lines and exploring how health care education might be improved to more effectively address the needs of these sectors. "The Blue Rose" are three sisters, Donna, Tina and Angela, who have been singing together for their entire lives. They were raised in a diverse musical family including their father, a published country/western songwriter, and their mother who sang and enjoyed all of the music from Broadway shows. She was also an active member of The Sweet Adelines. Their uncle was one of the original Texas Playboys and played "fiddle" for many of the name bands in that field and continues to be a vital part of the country music scene today. Needless to say, their harmony is influenced in some way by each of the different syles of music that they were introduced to, making their "gut harmony" truly unique.

Each of the sisters is a songwriter; one pop/country, one country, and one gospel. As children, the two eldest sisters sang regularly on a local program in Southern California and appeared on The Hometown Jamboree with Willie Nelson, Merle Travis and other country greats. After moving to Tulsa, Oklahoma, the youngest sister was added to the group, and the trio was born. They spent the next few years singing country, going to school, marrying and raising families. A chance request of a song from the 40's convinced them that they should be singing songs from that era. For the last several years they have been enthusiastically welcomed into their community by those who also enjoy this style of reminiscent music, and encourage "The Blue Roses" to "Keep it alive for our children."

In addition to their appearances locally, they will be performing at the 2004 Tulsa State Fair in the Grace Health Pavillion located in the IPE building. Performance dates are Thursday through Sunday, September 30th through Ocober 3rd. Shows begin at 6:30PM and 8:30PM.

The meeting was held in Withers Hall where mostly Turkish students from Duke, NCSU and University of North Carolina attended.

He was asking for help from the young crowd to assist them on their desire to improve the situation, as he said, this would be the only way to a stronger Turkey.

Specific Aim 1: Efficacy--symptom levels
To determine if symptoms of combat stress, depressive symptoms, and mental health burden decline after using the Vets Prevail intervention. Study organizers hypothesize that self reported symptoms and burden will be reduced significantly by week 8 compared to baseline.

Specific Aim 2: Satisfaction with internet training
To determine if the user perceived the internet training to be effectively conveyed. Study organizers hypothesize that users will rate training domains (internet program) in the strongly positive range.

The trial and research are being supported by the McCormick Foundation in partnership with Major League Baseball's "Welcome Back Veterans" initiative. Vets Prevail has also received financial support from the National Science Foundation's Small Business Innovation Research Program.
