Definition of Depression: Depression is a medical disorder characterized by feelings of sadness, hopelessness, pessimism, and a general loss of interest in life, combined with a sense of reduced emotional well-being.

Description of Depression:

The three (3) most common types of depression are major depression, dysthymia and bipolar disease:

Major depression is manifested by a combination of symptoms (see Symptoms below) that interfere with the ability to work, sleep, eat and enjoy once pleasurable activities. These symptoms occur without any apparent cause and deepen and persist day-in and day-out for two weeks or longer.

Dysthymia is marked by mild to moderate depressive symptoms that last at least two years. People with this condition have fewer symptoms than those with major depression, but the symptoms last longer and develop more slowly. On average, symptoms can last 16 years before there is a diagnosis. Sometimes people with dysthymia also experience major depressive episodes.

Bipolar disorder, formerly called manic-depressive illness, involves cycles of depression and elation (mania). Sometimes the mood switches are dramatic and rapid, but most often they are gradual. When in the depressed cycle, a person can have any or all of the symptoms of depression. When in the manic cycle, any or all manic symptoms (see Symptoms below) may be experienced.

Causes and Risk Factors of Depression:

Depression is caused by a combination of genetic (inherited), psychological and environmental factors. Depression, especially bipolar disorder, has been shown to run in families. Additionally, having too little or too much of certain neurochemicals in the brain have shown to be the cause of depression. The psychological makeup of a person, such as low self-esteem, pessimism and stress, can also cause depression. Environmental or other associated health factors, such as a serious loss, chronic illness, pregnancy, childbirth, difficult relationships, financial problems, unwelcome changes in life patterns, alcohol, drug abuse and medications can also trigger a depressive episode.

About 10 to 15 percent of all depression is caused by medical illness or medicine. Once these conditions are treated or the medication stopped or changed, the depression will usually go away.

The symptoms of depression include:

loss of interest in the things that the person once enjoyed feeling sad, blue, or down in the dumps feeling slowed down or restless and unable to sit still feeling worthless or guilty an increase or decrease in appetite or weight thoughts of death or suicide problems concentrating, thinking, remembering or making decisions trouble sleeping or sleeping too much loss of energy or feeling tired all of the time headaches digestive problems sexual problems feeling pessimistic or hopeless being anxious or worried

The symptoms of mania are:

feeling unusually "high," euphoric or irritable severe insomnia grandiose notions increased talking disconnected and racing thoughts increased sexual desire markedly increased energy poor judgment inappropriate social behavior

Diagnosis of Depression:

Diagnosis of depression involves a psychological/physical history and evaluation. According to the American Psychiatric Association, if a person experiences a loss of interest in the things they once enjoyed and are feeling sad, blue, or down in the dumps for at least two (2) weeks, and are experiencing at least five (5) of the other symptoms of depression, they may have major depression. If a person experiences euphoria, irritability, or a feeling of being "high" with four (4) other symptoms of mania for at least one (1) week, they may havebipolar disorder.

In addition to listing the symptoms, the doctor will also ask when the symptoms began, how long they have lasted, how severe they are, whether the individual has had them before, and if so, whether or not they were treated and what treatment was received, as well as whether or not other family members have had these symptoms.

Furthermore, a mental status examination will be done to determine if speech, thought patterns or memory have been affected. The physical examination will either diagnosis and/or rule out any medical conditions (such as thyroid disease, cancers or neurologic diseases) that could be causing the depression.

Treatment of Depression:

Treatment of depression usually involves medication, psychotherapy or a combination of both. Other treatments may include electroconvulsive therapy (ECT), light therapy and alternative treatments.

Medication:

There are more than 20 antidepressant drugs currently available. Antidepressants correct the chemical imbalance in the brain. Because a variety of drugs target different neurotransmitters and imbalances of these neurotransmitters can vary from patient to patient, some drugs may be more effective than others for any individual. Sometimes a combination of drugs is best.

There are four (4) groups of antidepressant medications most commonly used to treat depression:

Tricyclic antidepressants (TCAs), which include: amitriptyline (Elavil) imipramine (Trofanil, Janimine) nortryptyline (Pamelor) despiramine (Norpramin)

TCAs work by slowing the rate at which neurotransmitters (chemical messengers) re-enter brain cells. This increases the concentration of the neurotransmitters in the central nervous system which relieves depression.

Monoamine oxidase inhibitors (MAOIs) include phenelzine (Nardil) and tranylcypromine (Parnate). MAO is an enzyme responsible for breaking down certain neurotransmitters in the brain. MAOIs inhibit this enzyme and restore more normal mood states.

Lithium carbonates, including Eskalith and Lithobid. Lithium reduces excessive nerve activity in the brain by altering the chemical balance within certain nerve cells. This drug is effective is treating bipolar disorder.

Selective serotonin reuptake inhibitors (SSRIs) include: fluoxetine (Prozac)

Fluvoxamine (Luvox)

Paroxetine (Paxil)

Sertraline (Zoloft)

Citalopram (Celexa)

Escitalopram oxalate (Lexapro)

SSRIs act specifically on serotonin, making it more available for nerve cells, thus easing the transmission of messages without disrupting the chemistry of the brain. Two other antidepressants that affect two neurotransmitters, serotonin and norepinephrine, are venlafaxine (Effexor) and nefazodone (Serzone). Another of the newer antidepressants, bupropion (Wellbutrin), is chemically unrelated to the other antidepressants. It has more effect on norepinephrine and dopamine than on serotonin.

Medication usually produces a marked improvement by six weeks, but may require up to 12 weeks for full effect.

Psychotherapy:

Psychotherapy involves talking to family doctor, counselor or therapist about things that are occurring in a person's life. The aim of psychotherapy is to remove all symptoms of depression and return a person to a normal life.

There are three psychotherapies available to treat depression: behavioral therapy, cognitive therapy or interpersonal therapy. Behavioral therapy focuses on current behaviors, cognitive therapy focuses on thoughts and thinking patterns, and interpersonal therapy focuses on current relationships.

Although psychotherapy may begin to work right away, it may take eight to 10 weeks to show a full effect for some people.

Electroconvulsive therapy (ECT):

ECT, also called electroshock treatment, is used for severely depressed patients and/or those who have not responded to antidepressant medication and/or psychotherapy. During this therapy, an electric current travels through electrodes placed on the temples, causing a generalized shock that produces biochemical changes in the brain.

Light Therapy:

In light therapy, a special kind of light called a broad-spectrum light, is used to give people the effect of having a few extra hours of daylight each day. Light therapy is helpful in treating people with seasonal affective disorder (SAD). SAD is a serious depression that recurs each year at the same time, starting in fall or winter and ending in spring.

During the treatment process try these helpful tips:

Do not set difficult goals or take on a great deal of responsibility. Break large tasks into small ones, set priorities, and do what you can. Try to be with other people. Participate in activities that make you feel better. Do not overdo it or become upset if the treatment does not work right away. Do not make major life decisions. Do not expect to snap out of depression. Do not expect too much of the depressed person.

Alternative for Depression:

Light therapy, acupuncture, herbal remedies (such as St. John's Wort), exercise, mediation and fasting are shown to help people lessen or eliminate symptoms associated with depression.

Questions To Ask Your Doctor About Depression:

Has a complete diagnostic evaluation been given? What type of depression is it? What kind of treatment should be used or sought? Would an antidepressant medication be prescribed? If so, what are the side effects? After treatment is started, how long will it take before there is an improvement? Is the depression likely to return? If the depression is due to a serious loss, how long is the depression going to last? What measures can be taken to help the process? Where can the family get help to cope with this disorder or get more information?

"I remember waking up in the recovery room screaming in pain," said patient Kristina Henry of her six-hour surgery, followed by a month's recuperation. "When my doctor would come to check on me, I felt like I was a test monkey. There were quite a lot of doctors and students that saw parts of me that very few people have seen to this day. The whole experience was painful and unbearable."

Describing one physician's approach she said, "He thought I was going to go straight from surgery and start having sexual intercourse, so he took out my stent that was supposed to stay in, according to my original doctor. I was supposed to dilate and at 16 that was not a top priority for me. It hurt and I was so tired of pain. After time this made my length decrease and eventually it decreased to barely nothing. To this day I cannot wear a swimsuit without a cover-up because my scars are very noticeable."

Problem Often Undetected Until Puberty

Up to a half million women in the U. S. are born with anomalies of the urinary and reproductive system. In females thousands of these mullerian anomalies, including the absence of a vagina, may not be discovered until the girl reaches puberty.

MRKH (Mayer-Rokitansky-Kuster-Hauser) syndrome includes failure of the vaginal development, whether or not the uterus is present. Adolescent girls with MRKH syndrome and obstruction defects may complain that they have not begun menstruation, labeled primary amenorrhea.

In some cases, female patients have discovered their anomaly after inability to have intercourse because of an absent vagina. For many, multiple painful attempts of intercourse have led to distortion of existing external genital structures and scarring of the area, as well as chronic pelvic pain.

Laparoscopy for Vaginal Aplasia, or Non-Development of the Vagina

Techniques for correction over the years involved use of segments of the rectum, sigmoid colon and small intestine, as well as skin graft. In 1898, using open surgery, Dr. D. O. Ott formed a neovagina using peritoneum of the pelvis. It was then discovered that the cells of the peritoneum (the smooth transparent membrane lining the abdominal cavity) rapidly converted into typical vaginal cells. Different approaches to this technique evolved over time.

Nearly 100 years later, in 1993 Dr. L. V. Adamyan developed a laparoscopy-assisted technique for colpopoesis (creation of the neovagina), using the pelvic peritoneum. This minimally invasive technique is far easier on the patient, as it involves only tiny incisions and is performed in less than an hour. More than one thousand of them have been performed, proving their efficacy.

The procedure allows achievement of adequate functional vaginal length and elasticity, and the peritoneal epithelium (cellular layer that lines the walls of the abdominal cavity) converts into normal vaginal epithelium within three months after surgery.

World-renowned head of the Center for Women's Care & Reproductive Surgery, Thomas L. Lyons, M. D., and his Fellow, Assia A. Stepanian, M. D., learned the technique firsthand from its author, and they are the only U. S. surgeons with experience in laparoscopic correction of MRKH syndrome using the pelvic peritoneum.

This procedure was used to correct Kristina's post-surgical shortening of the vagina, which occurred from previous surgeries.

"It's very unfortunate that so many women have these suboptimal procedures because they often fail," said Dr. Lyons about the old style MacIndoe split thickness skin graft, which Kristina endured originally.

"Most academic centers still use the old way, which involves stretching skin over a glass tube stent and a great deal of 'hope' that the tissue will re-vascularize (grow new blood vessels) and stay healthy," he said. www. thomasllyons. com

Blessed with a loving husband who told her he didn't see her scars, Kristina wanted to have a normal sex life but had lost most of her depth.

As part of its rules for weight management, the National Federation of State High School Associations requires wrestlers to pass a hydration test. The PDX-CL checks an athlete's hydration by measuring his or her urine specific gravity. Cambridge University Press has adopted a giant panda at the Chengdu Giant Panda Breeding Research Foundation in China, in a bid to build closer working links with the country and to help protect this endangered species. Cambridge University Press has adopted a giant panda at the Chengdu Giant Panda Breeding Research Foundation in China, in a bid to build closer working links with the country and to help protect this endangered species. This is the first time a publisher has been granted access to a giant panda in this way.

Chief Executive Stephen Bourne, other members of staff from Cambridge’s offices and senior members of the Chinese State Council Information Office were all present at the adoption ceremony of the panda, named ‘Jian Qiao’, which can be loosely translated as ‘Cambridge’ in some Chinese dialects.

The association with Jian Qiao will help to raise the profile of the Cambridge Young Learners’ course books published for Chinese school children. As part of the adoption, a new web site will also be set up where children can follow the progress of Jian Qiao as she grows up.

The adoption of Jian Qiao also forms part of a wider conservation and climate change programme at the Press. Cambridge works with The Wildlife Trust in the UK to support them in their work, and staff are given paid time off to volunteer for charitable or environmental causes.

Giant pandas are native to China and they are among the world’s most endangered species. A 2007 report by the Chinese State Forestry Commission found that there were fewer than 2,000 giant pandas living in the wild today, and 239 pandas living in captivity in China.

Cambridge has a history of collaborating with the Chengdu Giant Panda Breeding Research Foundation. The Foundation’s Director, Professor Zhang Zhihe, was one of the expert contributors to the Cambridge book Giant Pandas - Biology, Veterinary Medicine and Management.

Cambridge also has a strong tradition of publishing about China, including titles in the 30-strong Cultural China series, as well as a growing presence in the English Language Teaching publishing market in the country.

Jian Qiao was born in August 2010, weighing 153 grams. Now weighing roughly four kilograms, she is doing well and has a healthy appetite.

Because pets have a penchant for chewing, items such as Christmas lights and other wire-based electrical decorations should be used sparingly. Candles should only be used when pets are secured or are closely being monitored. When away from your home, secure your pets away from these items. As an added precaution, unplug lights and motorized decorations.

Treating pets to people food

With the holidays comes the consumption of an inordinate amount of fatty, salty, sugary and calorically dense foods. Allowing pets to indulge in these same holiday excesses is not only bad for their health; it can lead to digestion problems and numerous other health issues.

As a rule of thumb, never feed people food to pets---especially treats. Common holiday foods like chocolate, raisins and macadamia nuts can cause serious physical and internal harm to a pet. Stray wrappers from candies can also be a problem since there is a potential for choking and/or intestinal blockage, if ingested.

Since removing the plethora of temptations is largely impractical, PSI suggests securing your pet in a "safe room" away from guests and potential hazards. After the festivities, be sure to properly discard all food scraps and trash before letting your pet back into the other areas of your home.

Forgetting to tell Santa about your pet

He's made his list. He's checked it twice. Nevertheless, forgetting to gift your pet is naughty...not nice. Offer to bring the packing boxes to the local recycling center or, if the newcomers have a pet, point out the other homes where there are dogs or other animals they can befriend. Don't gossip. It's very bad taste to talk about the old homeowners or other people in the neighborhood. Let the newcomers form their own opinions.
